Mechanism of action
Imodium is an antidiarrheal drug. Loperamide binds to opioid receptors in the intestinal wall. As a result, inhibition of the release of acetylcholine and prostaglandins occurs, which leads to a decrease in propulsive peristalsis and an increase in the time for the contents to pass through the intestines. Loperamide increases the tone of the anal sphincter, thus reducing incontinence and urge.
Symptomatic treatment of acute and chronic diarrhea;
Ileostomy (to reduce the frequency of bowel movements and stool volume, as well as to impart hardness to its consistency).
Contraindications
- acute dysentery, characterized by the presence of blood in the stool and high fever (Imodium should not be prescribed as the main therapy);
- acute ulcerative colitis or pseudomembranous colitis associated with taking broad-spectrum antibiotics;
- Imodium should not be used if it is necessary to avoid the suppression of peristalsis;
- constipation, bloating, subileus;
- children's age up to 6 years (due to the high dosage of capsules for this category of patients);
- Hypersensitivity to the drug.
Pregnancy and Breastfeeding
In pregnancy, especially in the first trimester, Imodium can be prescribed in cases where the expected benefit of therapy for the mother outweighs the potential risk to the fetus.
Data on the teratogenic or embryotoxic effects of Imodium are not available.
Data on the allocation of loperamide with breast milk is limited. Since a small amount of the drug was found in breast milk, taking Imodium during breastfeeding is not recommended.
Dosage and administration
Inside: the lingual pill is put on the tongue, within a few seconds it disintegrates, after which it is swallowed with saliva, without drinking water.
Initial dose: for adults - 2 tabl.; for children over 6 years old - 1 tab.
With acute diarrhea - 1 tab. after each liquid stool;
with chronic diarrhea The dose is adjusted to reduce the number of acts of defecation with a fixed stool up to 1-2 times per day (the supporting daily dose is usually 1-6 tables.).
The maximum daily dose (with diarrhea) adults - 8 tab., in children - 1 tab. on 7 kg.
Adverse reactions
Gastrointestinal: constipation and / or bloating, in very rare cases associated with intestinal obstruction, especially if the dosing regimen is not followed.
Dermatologic: rash.
Other: symptoms that are usually difficult to distinguish from those associated with diarrhea syndrome — abdominal pain or discomfort, nausea and vomiting, fatigue, drowsiness, or dizziness; dry mouth.
In none of the cases when the recommendations for use were observed and the contraindications were taken into account, there were no serious adverse reactions, even after a long reception.
Special notes
Since Imodium is a drug for symptomatic therapy, if possible, it is also necessary to prescribe etiotropic therapy.
In diarrhea, especially in children, hypovolemia and a decrease in the content of electrolytes are possible. In such cases, replacement therapy is the most important for replenishing fluids and electrolytes.
In acute diarrhea, if there is no clinical improvement within 48 hours, stop taking Imodium and consult a doctor.
Since loperamide is characterized by a high degree of metabolism during the "first pass" through the liver, in patients with liver dysfunction it is possible to slow down and increase the concentration of loperamide in the blood plasma. This category of patients requires careful medical observation to identify symptoms of toxic CNS damage under the influence of loperamide.
With the development of constipation, abdominal distention or subileus, you should immediately stop taking Imodium.
Use in pediatrics
Imodium should not be prescribed to children under the age of 6 years without the supervision of a physician.
Influence on ability to drive motor transport and control mechanisms
Imodium does not interfere with the speed of the reaction. However, in the event of fatigue, drowsiness or dizziness, it is not recommended to drive vehicles and work with machinery.
