Composition

Pektrol - pills of the prolonged action:

1 pill contains isosorbid-5-mononitrate 40 and 60 mg; 30 and 100 pieces per pack.

Mechanism of action

Peripheral vasodilator with a predominant effect on the venous vessels. Pectrol stimulates the formation of nitric oxide (endothelial relaxing factor) in the vascular endothelium, which causes the activation of intracellular guanylate cyclase, resulting in an increase in cGMP (vasodilation mediator). Vasodilatation reduces venous return to the heart (preload), reducing the load on the heart, which reduces the need for myocardial oxygen. The dilating effect of nitrates on arteries and arterioles reduces the round-offs (afterload). Pektrol has a coronary dilating effect. Reduces blood flow to the right atrium, helps reduce pressure in the pulmonary circulation and regression of symptoms in pulmonary edema. It promotes the redistribution of coronary blood flow in the area with reduced blood supply. Pektrol raises tolerance to an exercise stress at patients with an ischemic heart disease, stenocardia. Expands the vessels of the brain, dura mater, which may be accompanied by headache. Inhibits platelet aggregation, reduces intraplatelet synthesis of thromboxane. Like other nitrates, cross-tolerance develops.After cancellation (interruption of treatment), sensitivity is quickly restored. Antianginal effect occurs 30-40 minutes after ingestion and lasts up to 8-10 hours.

Contraindications

hypersensitivity to nitrates or other components of Pectrol; arterial hypotension and hypovolemia (MAP <100 mm Hg., DAD <60 mm Hg., central venous pressure <4-5 mm Hg); acute heart failure; Shock; age up to 18 years (efficacy and safety have not been established).

Adverse reactions

From the side of the cardiovascular system: “nitrate” headache, dizziness, transient flushing of the skin of the face, sensation of heat, tachycardia, marked decrease in blood pressure; in rare cases - increased angina attacks (paradoxical reaction), orthostatic collapse. On the side of the gastrointestinal tract: nausea, vomiting, a slight burning sensation of the tongue, dry mouth can appear. On the CNS side: stiffness, drowsiness, blurred vision,decrease in the ability to rapid mental and motor reactions (especially at the beginning of treatment); in rare cases - cerebral ischemia. Allergic reactions: skin rash, in some cases - exfoliative dermatitis. Others: the development of tolerance (including cross to other nitrates).

Dosage and administration

Pektrol accept inside, after food, swallowing entirely and washing down with a glass of water. The usual initial dose is 40 mg 1 time per day (in the morning). The dose of the drug can be increased up to 60 mg once a day or up to 80 mg (40 mg twice a day).

Pectrol is not intended to relieve an attack of angina. Correct use of isosorbide mononitrate ensures a period with a low concentration of nitrate, which is necessary to prevent the development of tolerance to nitrates. As with all nitrates, it is important to strictly follow the instructions for dispensing Pectrol and observe the 12-hour interval between doses. In this case, a period with a low concentration (<100 ng / l) is achieved, and the therapeutic effect is maintained.
