Composition
One gram of cream contains 0.64 mg betamethasone dipropionate (equivalent to 0.5 mg betamethasone) and Gentamicin sulfate - 1 mg.

Mechanism of action
Combined preparation for external use, has anti-inflammatory, anti-allergic and anti-bacterial action. Betamethasone dipropionate - glucocorticosteroid, has anti-inflammatory, anti-allergic, anti-exudative, anti-edema and antipruritic action. It inhibits the accumulation of leukocytes, the release of lysosomal enzymes and proinflammatory mediators in the focus of inflammation, inhibits phagocytosis, reduces vascular tissue permeability, prevents the formation of inflammatory edema. Gentamicin is a broad-spectrum antibiotic from the group of aminoglycosides. It has a bactericidal effect against pathogens of primary and secondary bacterial infections of the skin. Active against gram-negative bacteria: Pseudomonas aeruginosa, Aerobacter aerogenes, Escherichia coli, Proteus vulgaris, Klebsiella pneumoniae; Gram-positive bacteria: Streptococcus spp.(sensitive strains of beta and alpha hemolytic streptococcus group A), Staphytococcus spp. (coagulase-positive, coagulase-negative and some strains producing penicillinase). Inactive against anaerobes, fungi and viruses.
Indications and usage
Treatment of steroid-sensitive dermatoses, complicated by primary and / or secondary bacterial infection, eczema (atopic, pediatric, mono-type), limited and diffuse allergic dermatoses, including those complicated by gentamicin-sensitive infection, dermatitis (simple and allergic), atopic dermatitis, sunny dermatitis, exfoliative and radiation dermatitis, diaper rash, psoriasis, pruritus.
Dosage and administration
The drug Akriderm® GENTA is applied with a thin layer on the inflamed surface twice a day, slightly rubbing. In milder cases, single use is sufficient. The duration of treatment is determined individually and depends on the nosological form and severity of the disease and is usually 1-2 (rarely 4 weeks).
If necessary, a long course of therapy is recommended to use the drug less often, for example, every other day.
In the treatment of chronic diseases under the constant supervision of a physician, treatment should continue for some time after the disappearance of all symptoms in order to avoid recurrence of the disease.
The use of the drug in children under 12 years of age is carried out under the supervision of the attending physician.
Contraindications
Hypersensitivity to the drug.Skin tuberculosis, viral skin infections, vaccinations, perioral dermatitis, rosacea (rosacea).
Adverse reactions
Burning, dry skin, folliculitis, hypertrichosis, hypopigmentation, allergic contact dermatitis, skin atrophy, stretch marks, prickly heat.
In the absence of the effect of treatment within 2 weeks, you should consult a doctor for a possible clarification of the diagnosis. If the use of the drug marked irritation or hypersensitivity reaction, treatment should be stopped and consult a doctor. For secondary fungal or bacterial infection, it is necessary to apply the appropriate drugs. In the absence of a quick positive effect, the use of Acriderma should be discontinued until all signs of infection have been eliminated. Not recommended long-term use of the drug on the skin of the face. Do not use the drug in the eye area (possibly the development of cataracts, glaucoma, fungal infection and exacerbation of herpes infection). The use of the drug in children under 12 years should be under the supervision of the attending physician. Children are more susceptible to the use of local glucocorticoids, which cause inhibition of the hypothalamic-pituitary-adrenal system, than older patients, due to the increased absorption of the drug, associated with a higher ratio of their surface area and body weight.In children who took local glucocorticoids, the following side effects were noted: suppression of the hypothalamic-pituitary-adrenal system, Cushing syndrome, linear growth retardation, lag in weight gain, increased intracranial pressure. Symptoms of inhibition of the function of the adrenal cortex include a decrease in plasma cortisol and a lack of response to stimulation of ACTH. Increased intracranial pressure is manifested by bulging of the fontanel, headache, bilateral swelling of the optic nerve head.
Pregnancy and Breastfeeding
Perhaps if the expected effect of therapy for the mother outweighs the potential risk to the fetus. Contraindicated in pregnancy in large doses and for a long time. At the time of treatment should stop breastfeeding.
Special notes
The drug is not used in ophthalmology. Avoid getting the cream in the eyes, on damaged skin and open wound sites. In order to increase the effectiveness of therapy it is possible to use occlusive dressings. It must be borne in mind that this increases the transdermal absorption of betamethasone and gentamicin, which leads to an increased risk of systemic side effects. When a persistent bacterial or fungal microflora appears, the use of the drug should be discontinued and appropriate therapy should be prescribed.possible development of cross-resistance with aminoglycoside antibiotics.
