Composition

One tabletTriampur Compositum contains active ingredients: 25 mg of triamterene and 12.5 mg of hydrochlorothiazide.
Excipients: lactose monohydrate, potato starch, polyvidone K 25, highly dispersed silicon dioxide, sodium starch glycolic acid, Magnesium stearate;

- arterial hypertension;
- edema caused by diseases of the heart, liver or kidneys, mainly in cases where it is necessary to avoid the loss of potassium by the body
- the drug is used in combination with cardiac glycosides in case of heart failure, if additional removal of fluid and reduction of potassium excretion is required

Contraindications

Hypersensitivity to active substances contained in the preparation or to sulfonamides; severe renal dysfunction, severely limited urine formation (anuria), acute kidney inflammation (acute glomerulonephritis), severe liver dysfunction (precoma and hepatic coma), electrolyte imbalance (elevated potassium or Calcium in plasma, very low potassium or sodium in blood plasma); hypovolemia.

Pregnancy and Breastfeeding

Triampur compositum is contraindicated for use during pregnancy and lactation.
If necessary, treatment for vital reasons stop breastfeeding.

Dosage and administration

For adults and adolescents weighing more than 50 kg, the following dosage regimens are usually valid:
Edemas:
At the beginning of treatment, 2 pills are prescribed. Triampur Compositum morning and noonuntil the moment of release of water from the body. Maintenance dose is 1 pill per day, which is taken every day in the morning, or 2 pills every second day (1 pill in the morning and at noon). If necessary, this maintenance dose can be increased to 4 pills per day (2 pills in the morning and at noon).
Hypertonic disease:
Initial doseTriampur Compositum - 2 pills in the morning and 2 pills at noon. With the addition of other antihypertensive drugs or for long-term therapy, 2 pills per day are sufficient.
Concomitant therapy in the treatment of heart failure with glycosides:
Assign 2 pills per day, if necessary, give a maximum of 4 pills per day.
With reduced kidney function (serum creatinine 1.5-1.8 mg / dL or creatinine clearance 50-30 ml / min) should not exceed the daily dose of 1 tablet.

Adverse reactions

From the nervous system and sensory organs: sometimes - ataxia, drowsiness, dry mouth, thirst, fatigue, headache, nervousness; rarely, mild visual disturbances, worsening of existing myopia, reduction of lacrimal secretion, etc.

Metabolism: electrolyte imbalance, water metabolism, metabolic acidosis, increased cholesterol, triglycerides, glucose, uric acid in the blood; dehydration, hypovolemia.
Musculoskeletal system: lethargy of skeletal muscles, muscle tension, cramps of the gastrocnemius muscles.
Allergic reactions: in rare cases - redness and itching of the skin, urticaria, erythematosus, rash, fever.

Overdosage

Symptoms: frequent urge to urinate, weakness, fatigue, states of confusion, crawling in hands and feet, a decrease in arterial pressure, heart rhythm disturbances, and sometimes convulsions.
Treatment: induce vomiting or flushing of the stomach.

When conducting long-term treatment with triampur compositum, it is necessary to periodically monitor certain laboratory parameters of the blood (residual nitrogen, electrolyte concentration, in particular the content of potassium, creatinine, lipid, sugar and uric acid). Especially in the elderly or with simultaneous treatment of cardiac glycosides, adrenal cortex hormones (glucocorticoids, such as betamethasone, prednisolone, triamcinolone) or laxatives, the concentration of potassium, creatinine and glucose in the blood is determined more often.

If you suspect a lack of folic acid in the body (vitamin B deficiency, for example, alcoholic cirrhosis of the liver), regular blood tests should be performed.

Before examining the function of the parathyroid gland and for at least three days before the test with the sugar load, the triampure treatment is interrupted, as otherwise false indicators can be obtained.

Antihypertensive drugs (especially ACE inhibitors), diuretics, barbiturates, phenothiazines, tricyclic antidepressants, vasodilators enhance the hypotensive effect; NSAIDs - weaken.
Indomethacin with simultaneous use reduces glomerular filtration, Kolestiramin reduces absorption, potassium salts, potassium-saving diuretics, ACE inhibitors increase the risk of hyperkalemia, glucocorticoids and laxatives - hypokalemia.
Weakens the effect of antidiabetic, anti-arthritic drugs, norepinephrine and adrenaline, enhances the cardio-and neurotoxic effect of lithium, side effects of salicylates on the central nervous system; enhances the effect of curare-like muscle relaxants, cardiac glycosides (if there is a deficiency of potassium and / or magnesium in the body), cytostatics (on the bone marrow); reduces quinidine clearance.

The antihypertensive effect of triampur compositum can increase alcohol, so avoid excessive drinking.
