Mechanism of action

Angiotensin-converting enzyme inhibitor. Inhibits the formation of angiotensin II and eliminates its vasoconstrictor effect on arterial and venous vessels. Reduces total peripheral vascular resistance, reduces afterload, reduces blood pressure. Reduces preload, reduces pressure in the right atrium and the pulmonary circulation. Reduces the formation of aldosterone in the adrenal glands.

After oral administration, 60-75% of the drug is absorbed from the gastrointestinal tract.
Communication with protein 30%.
The half-life of the drug is 2-3 hours.
The drug is excreted mainly by the kidneys, about 50% - unchanged.
Capoten is excreted in breast milk.

Indications and usage

• arterial hypertension (including renovascular);
• chronic heart failure (as part of complex therapy).

The dose of the drug is set individually in each case.
In the treatment of arterial hypertension the initial dose is 12.5 mg 2 times a day. If necessary, gradually increase the dose of the drug (with an interval of 2-4 weeks). In case of arterial hypertension of mild and moderate severity, the maintenance dose is 25 mg 2 times a day, maximum - 100 mg 2 times a day.In severe form of arterial hypertension, maintenance dose is 50-100 mg 2 times a day, maximum 150 mg 2 times a day.
In the treatment of chronic heart failure The initial dose is 6.25-12.5 mg 3 times a day. If necessary, under the supervision of a physician, the dose of the drug can be increased to 25 mg 3 times a day.
The drug is taken on an empty stomach, 1 hour before meals.

Adverse reactions

Possible: dizziness, headache, hypotension, palpitations, fatigue, nausea, cough, asthenia.
Rarely: allergic reactions, tachycardia, abdominal pain, diarrhea, deterioration of kidney function.

Contraindications

• severe renal impairment;
• renal artery stenosis;
• hyperkalemia;
• pregnancy;
• lactation;
• childhood;
• hypersensitivity to the drug.

Special notes

With caution prescribed to patients with severe renal impairment, patients suffering from collagen diseases.
Patients with kidney disease and patients taking high doses of the drug should regularly monitor the presence of protein in the urine.
In order to reduce the risk of developing symptomatic hypotension, it is recommended to start treatment with small doses and take Kapoten overnight.
The first day of taking the drug in elderly patients is recommended in inpatient conditions.
AT gestation period the drug is not used.Kapoten causes a deficiency of amniotic fluid in pregnant women, which leads to the development of arterial hypotension and renal failure in the fetus.
Women of childbearing age, Capoten recipients should use effective contraceptive methods.
Capoten can contribute to a temporary increase in the level of nitrogen in the blood and urine, as well as serum creatinine in patients with reduced BCC and kidney disease.

Drug Interactions

Diuretics enhance the hypotensive effect of Capoten.
Potassium-sparing diuretics with simultaneous appointment with Capoten contribute to the development of hyperkalemia.
Non-steroidal anti-inflammatory drugs may reduce the hypotensive effect of the drug.
There are reports of the development of neutropenia and Stevens-Johnson syndrome in patients taking Capoten in combination with Allopurinol.
A number of patients with diabetes who take insulin or oral hypoglycemic drugs simultaneously with Capoten observed hypoglycemia.
Probenecid reduces capoten's renal clearance.
