Pharmacology

Ofloxacin - broad-spectrum antimicrobial agent from the group of fluoroquinolones, acts on the bacterial enzyme DNA-girazu, providing supercoiling and, thus, the stability of bacterial DNA (destabilization of DNA chains leads to their death).
It has a bactericidal effect.
It is active against microorganisms that produce beta-lactamase and fast-growing atypical bacteria. Sensitive: Staphylococcus aureus, Staphyiocqccus epidermidis, Neisseria gonorrhoese, Neisseria meningitidis, Escherichia coli, Citrobacier, Klebsiella spp. (including Klebsielia pneumonia), Enterobacter spp., Hafnia, Proteus spp. (including Proteus mirabilis, Proteus vulgaris - indole-positive and indole-negative), Salmonella spp, Shigella spp. (including Shigella sonnei), Yersinia enterocolitica, Campilobacter jejuni, Aeromonas hydrophila, Plesiomonas aeruginosa, Vibrio cholerae, Vibrio parahaemolyticus, Haemophilus influenzae, Chlamydia spp., Legionella s. Bordetella pertussis. Moraxella catarrhalis, Propionibacterium acnes. Staphylococcusspp. Brucella spp. Different sensitivity to the drug have: Enterococcus faecalis. In most cases insensitive.: Nocardia asteroides, anaerobic bacteria (Bacteroides spp., Peptococcus spp., Peptostreptococcus spp., Eubacterium spp., Fusobacterium spp., Clostridium difficile). He acts on Treponemapallidum.

Indications and usage

Respiratory tract infections (bronchitis, pneumonia}, ENT organs (sinusitis, pharyngitis, otitis media, laryngitis), skin, soft tissues, bones, joints, infectious and inflammatory diseases of the abdominal cavity and biliary ducts (except for bacterial enteritis), kidney (pyelonephritis), urinary tract (cystitis, urethritis), pelvic organs (endometritis, salpingitis, oophoritis, cervicitis, parametritis, prostatitis), genital organs (colpitis, orchitis, epididymitis), gonorrhea, chlamydia; septicemia (for intravenous administration only), meningitis; prevention of infections in patients with impaired immune status (including neutropenia).

Contraindications

Hypersensitivity, deicite glucose-6-phosphate dehydrogenase, epilepsy (including a history), reduction of seizure threshold (including after a traumatic brain injury, stroke or inflammatory processes in the central nervous system); age up to 18 years (skeletal growth is not yet completed), pregnancy, lactation period.
With caution - cerebral arteriosclerosis, cerebrovascular accident (history), chronic renal failure, organic damage to the central nervous system.

Dosage and administration

Pills Ofloxacin accept inside entirely, washing down with water to or during food. Doses are selected individually depending on the location and severity of the infection, as well as the sensitivity of microorganisms, the general condition of the patient and liver and kidney function.
Adults - 200-800 mg per day, the course of treatment is 7-10 days, the frequency of use - 2 times a day.A dose of up to 400 mg per day can be prescribed in 1 reception, preferably in the morning. With gonorrhea - 400 mg once.

The duration of the course of treatment is determined by the sensitivity of the pathogen and the clinical picture; treatment should continue for at least 3 days after the disappearance of symptoms and full normalization of temperature. In the treatment of salmonellosis, the course of treatment is 7-8 days, with uncomplicated infections of the lower urinary tract infections - 3-5 days.

Adverse reactions

On the part of the digestive system: abdominal pain, increased activity of liver transaminases, hyperbilirubinemia, cholestatic jaundice, pseudomembranous enterocolitis.
Nervous system: headache, dizziness, uncertainty of movements, tremor, convulsions, numbness and paresthesias of the extremities, intense dreams, “nightmarish” dreams, psychotic reactions, anxiety, agitation, phobias, depression, confusion, hallucinations, increased intracranial pressure.
Musculoskeletal system: tendonitis, myalgia, arthralgia, tendinovinit, tendon rupture.
Special senses: violation of color perception, diplopia, violation of taste, smell, hearing and balance.
From the CCC: tachycardia, a decrease in blood pressure (with a / in the introduction; with a sharp decrease in blood pressure, the introduction is stopped), you cool, collapse.
Allergic reactions: skin rash, pruritus, urticaria, allergic pneumonitis, allergic nephritis, eosinophilia, fever, angioedema, bronchospasm, Stevens-Johnson and Lyell syndrome, photosensitization, erythema multiforme, rarely anaphylactic shock.
From the skin: point hemorrhages (petechiae), bullous hemorrhagic dermatitis, papular rash with a crust, indicating vascular lesions (vasculitis).
From the side of blood-forming organs: leukopenia, agranulocytosis, anemia, thrombocytopenia, pancytopenia, interstitial nephritis, impaired renal function, hypercreatininemia, increased urea.
Other: dysbacteriosis, superinfection, hypoglycemia (in patients with diabetes mellitus), vaginitis.

Special notes

Not a drug of choice for pneumonia caused by pneumococci. Not indicated for the treatment of acute tonsillitis.
It is not recommended to use for more than 2 months, to be exposed to sunlight, exposure to UV rays (mercury-quartz lamps, solarium).
