Composition
Cream white or almost white
1 g of cream contains 0.64 mg betamethasone dipropionate (equivalent to 0.5 mg betamethasone); 10 mg of Clotrimazole; 1 mg of Gentamicin sulfate;Excipients: petrolatum, propylene glycol, medical vaseline oil, cetostearyl alcohol, macrogol cetostearyl ether, Trilon B, sodium dihydrate phosphate monosubstituted, purified water.
Mechanism of action
Akriderm GK - combined preparation for external use with anti-inflammatory, antiallergic, antifungal and antibacterial action.
Betamethasone has anti-inflammatory, anti-allergic, anti-exudative and antipruritic action. It inhibits the accumulation of leukocytes, the release of lysosomal enzymes and inflammatory mediators in the lesion, inhibits phagocytosis, reduces vascular tissue permeability, prevents the formation of inflammatory edema.
Clotrimazole has antifungal effect due to a violation of the synthesis of ergosterol, which is an integral part of the cell membrane of fungi. Active against mushrooms Trichophyton rubrum, Trichophyton mentagrophytes, Epidermophyton floccosum, Microsporum canis, Candida albicans, Malassezia furtur (Pityrosporum orbiculare).
Gentamicin is a broad-spectrum antibiotic of bactericidal action from the group of aminoglycosides, active against gram-negative bacteria Pseudomonas aeruginosa, Aerobacter aerogenes, Escherichia coli, Proteus vulgaris, Klebsiella pneumoniae; gram-positive bacteria Streptococcus spp., Staphylococcus aureus.
Indications and usage
- dermatosis, complicated by primary and / or secondary infection;
- pityriasis versicolor;
- ringworm (including inguinal, feet);
- limited or diffuse atopic dermatitis.
Dosage and administration
Acryderm HA is applied with a thin layer to the affected skin 2 times / day (morning and evening). The duration of treatment depends on the effectiveness and tolerability of therapy and is usually not more than 2-4 weeks.
Adverse reactions
Allergic reactions: urticaria, edema, erythema, allergic contact dermatitis.
Dermatologic: perioral dermatitis, acne, telangiectasia, pyoderma, striae, hypertrichosis, folliculitis, furunculosis.
Local reactions: burning sensation, hyperemia, exudation, itching, dry skin, peeling, pigmentation disturbance, skin maceration, skin atrophy, prickly heat.
Other: paresthesia. With prolonged use may develop systemic side effects of GCS (depression of the function of the adrenal cortex).
Contraindications
- skin tuberculosis;
- Syphilis (skin manifestations);
- chickenpox;
- herpes simplex;
- skin reactions after vaccination;
- open wounds;
- perforation of the eardrum (in the treatment of otitis externa);
- lactation period;
- children's age up to 1 year;
- hypersensitivity to the drug.
Pregnancy and breastfeeding
The safety of topical administration of GCS in pregnant women has not been established. The prescription of drugs in this group is justified only if the potential benefit exceeds the possible risk. During pregnancy, this group of drugs should not be used in high doses or for a long time.
It has not been established whether topical application of GCS can cause them to appear in mother’s milk, therefore the issue of discontinuing breastfeeding should be resolved.
The drug is not intended for use in ophthalmology. Avoid getting the cream in the eyes, on damaged skin and open wound sites.
In order to increase the effectiveness of therapy it is possible to use occlusive dressings. It must be borne in mind that this increases the transdermal absorption of betamethasone and gentamicin, which leads to an increased risk of systemic side effects.
When a persistent bacterial or fungal microflora appears, the use of the drug should be discontinued and appropriate therapy should be prescribed.
Perhaps the development of cross-resistance with antibiotics from the group of aminoglycosides.
Use in pediatrics
Children aged 1 to 16 years the drug is prescribed only under strict indications and under the supervision of a physician, since it is possible the development of systemic side effects associated with taking betamethasone and gentamicin.It should be borne in mind that when using the drug on large surfaces and / or under the occlusive dressing, it is possible to suppress the function of the hypothalamic-pituitary-adrenal system and develop symptoms of hypercorticism, as well as reduce the excretion of growth hormone and increase intracranial pressure.
Overdosage
Symptoms: possible development of symptoms of hypercortisolism: weight gain, edema, hypertension, glucosuria, hypokalemia.
Treatment: symptomatic therapy, if necessary - correction of water and electrolyte balance. Gradual withdrawal of the drug is recommended.
Storage conditions
