Mechanism of action

Glutoxim immunomodulator with systemic cytoprotective action. Glutoxim plays an important role in the regulation of metabolic processes in cells and tissues. It has a selective effect on the sulfhydryl groups of cell surface and soluble receptors, which leads to the restoration of their functional active conformation, sensitivity to regulatory transport molecules of peptide nature, determines the immunomodulatory and cytotropic effects of the drug. Glutoxim contributes to the implementation of the effect of regulatory molecules of peptide nature on normal (regulation of metabolic processes) and transformed (induction of programmed cell death) cells. The main immunophysiological properties of the drug include: high tropism to the cells of the central organs of the immune system and the lymphoid tissue system; increased bone marrow hematopoiesis, erythropoiesis, lymphopoiesis and granulocyto-monopoiesis; activation of the phagocytosis system (incl.in conditions of immunodeficiency), restoration in the peripheral blood levels of neutrophils, monocytes, lymphocytes and functional activity of tissue macrophages. The immunobiochemical effects of the drug include: the stimulating effect of the drug on the cascade mechanisms of phosphate modification of key proteins of the signal-transmitting systems; the initiation of the action of the cytokine system, (including interleukin 1, interleukin 2, interleukin 6, tumor necrosis factor, Interferon, erythropoietin).

Indications and usage

- Prevention and treatment of secondary immunodeficiency states associated with radiation, chemical and infectious factors.
- Restoration of suppressed immune reactions and depression of bone marrow hematopoiesis.
- Increasing the body's resistance to various influences (infection, intoxication, radiation).
- As a hepatoprotective agent for acute and chronic hepatitis B and C (as part of complex therapy).
- Potentiation of therapeutic effects of antibacterial therapy of chronic obstructive pulmonary diseases.
- Severe, common forms of tuberculosis of various localization (as part of complex therapy).
- Tuberculosis resistant to drug therapy.
- Treatment of toxic complications of anti-tuberculosis therapy.
- Prevention of exacerbations of chronic hepatitis in tuberculosis patients on the background of anti-tuberculosis therapy.
- Prevention of postoperative purulent complications.

Contraindications

- Pregnancy.
- Lactation (breastfeeding).
- Hypersensitivity to the drug.

Pregnancy and Breastfeeding

Clinical studies of the drug Glutoxim during pregnancy and lactation (breastfeeding) were not conducted.

Dosage and administration

Adults inject the drug in / in / m and s / c in a dose of 5-40 mg daily, depending on the severity of the disease. Course dose - 50-300 mg. An isotonic solution of sodium chloride or 5% dextrose (glucose) solution should be used as a solvent for infusion administration of the drug.
With tuberculosis (as part of complex therapy) the drug is prescribed in / in or in the / m 30 mg 2 times / day, daily for 2 months. After the transition of a specific inflammation into the productive phase, the drug is administered i / m at a daily dose of 10–20 mg 1–2 times / day (morning and evening) 3 times a week. If necessary, conduct a second course of treatment after 1-6 months.
With psoriasis (as part of complex therapy) the drug is administered intramuscularly daily in a daily dose of 10 mg for 15 days, then for another 5 weeks 2 times a week in a daily dose of 10 mg. The course of treatment is 25 injections.
With the preventive purpose the drug is used in / m in a dose of 5-10 mg daily for 2 weeks.

Adverse reactions

Maybe: in some cases - an increase in body temperature (37.1-37.5 ° C), pain at the injection site (to reduce pain, it is possible to administer the drug simultaneously with 1-2 ml of a 0.5% solution of procaine / Novocaine /).

Special notes

An open, randomized clinical trial of the drug Glutoxim in the complex therapy of drug-sensitive and drug-resistant tuberculosis in the long (52 days) and shortened mode (24 days) demonstrated identical efficacy and tolerability of the drug. Glutoxim administration for 24 days (17 injections), similar to 52 days (104 injections), resulted in a reduction in the time of disappearance of intoxication, time to achieve negativity of sputum and the reverse development of the inflammatory process in the lungs. Similarly, the 17-fold administration of Glutoxim allowed in a short time to prepare patients for surgery and prevent postoperative complications. Glutoxim treatment in shorter periods also prevents the exacerbation of chronic hepatitis in patients with tuberculosis, eliminates the symptoms of drug-induced liver damage, normalizes the menstrual cycle and increases the potency, a decrease caused by the side effects of anti-TB drugs.
The results of recent clinical studies indicate the possibility of using Glutoxim as a drug for immunomodulating support therapy in cases of immunological disorders caused by a combination of recurrent sexually transmitted infections (STIs) and antibiotic-resistant forms of chronic bacterial prostatitis (CKD). Glutoxim stimulates the process of removing parasitic microorganisms from cells intracellularly.
Glutoxim increases the effectiveness of antibacterial drugs, in particular the quinolone series, most often used in urological practice. The use of Glutoxim in the complex therapy of CKD in combination with an STI allows to increase the duration of remission of the disease, the absence of relapse in a significant number of patients. The high level of the cytoprotective effect of Glutoxim increases the resistance of normal cells to the effects of Chemotherapy, which explains the reduction in the side effects of antibiotics and the absence of allergic reactions. The inclusion of Glutoxim in CKD treatment regimens contributes to the improvement of the male and female ereculatory and ejaculatory components of the copulatory cycle.
