Mechanism of action

Vinpocetine - a drug that improves cerebral circulation. It has a vasodilator, antihypoxic and antiplatelet effect. The mechanism of action is associated with inhibition of phosphodiesterase, which leads to an increase in the content of cAMP in the cells, which, in turn, causes a decrease in the Calcium content in the cytoplasm of smooth muscle cells and the relaxation of myofibrils.
It expands the blood vessels of the brain, increases blood flow, mainly in ischemic areas, improves the supply of oxygen to the brain. It promotes glucose utilization and increases the level of catecholamines in the central nervous system, stimulates the metabolism of norepinephrine and serotonin in brain tissues. Reduces platelet aggregation, blood viscosity, contributes to increased red blood cell deformability and normalization of venous outflow against the background of decreasing resistance of cerebral vessels. Systemic blood pressure drops slightly.
Vinpocetine is effective in the acute period of stroke: it accelerates the regression of cerebral and focal neurological symptoms, improves memory, attention, and intellectual productivity. In the elderly and senile age, the sensitivity of cerebral vessels to the relaxing effect of vinpocetine increases, which is due to the sensitization of the adenylate cyclase-cAMP system with aging.

Indications and usage

- acute (if parenteral administration is impossible) and chronic forms of cerebral circulatory insufficiency (including acute and residual stages of stroke, transient ischemic attacks, encephalopathy, dementia after cerebral circulatory disorders);
- vascular diseases of the retina and / or choroid (due to atherosclerosis, angiospasm, thrombosis); degenerative changes in macula caused by atherosclerosis or angiospasm; secondary glaucoma (due to vascular obstruction);
- age-related vascular or toxic (medical) hearing loss, Meniere's disease, labyrinth-related vertigo;
- vegetative manifestations of menopausal syndrome.

Contraindications

- pronounced cardiac arrhythmias;
- IHD (severe course);
- acute stage of hemorrhagic stroke;
- pregnancy;
- lactation (breastfeeding);
- Hypersensitivity to vinpocetine.

Pregnancy and Breastfeeding

Vinpocetine is contraindicated for use during pregnancy and lactation (breastfeeding).

Dosage and administration

IV drip. 20 mg is diluted in 250-400 ml of infusion solution. With good tolerance, the dose of the drug within 3–4 days is increased 1 mg / kg, the treatment is continued for 10–14 days. After completing the course of parenteral administration, the drug is taken orally. Before the abolition of the drug dose should be gradually reduced.

Adverse reactions

Dizziness, feeling of heat, flushing of the skin of the face, nausea, thrombophlebitis at the injection site. On the part of the cardiovascular system, a transient decrease in arterial pressure, tachycardia, extrasystole, and an increase in the time of excitation of the ventricles may be observed.

Drug Interactions

Increases the risk of hemorrhagic complications on the background of Heparin therapy.
Vinpocetine solution is pharmaceutically incompatible with heparin.

Overdosage
