Mechanism of action

Pharmacological action - anti-inflammatory, antiallergic, glucocorticoid.

Pharmacodynamics

Betamethasone dipropionate is a synthetic corticosteroids with anti-inflammatory, antiallergic, anti-exudative, antipruritic and vasoconstrictor effects. When applied to the surface of the skin narrows blood vessels, relieves itching, reduces the release of inflammatory mediators (from eosinophils and mast cells), IL-1 and -2, gamma Interferon (from lymphocytes and macrophages), inhibits activity and reduces the permeability of the vascular wall. It interacts with specific receptors in the cytoplasm of the cell, stimulates the synthesis of messenger RNA, inducing the formation of proteins, including lipocortin mediating cellular effects. Lipocortin inhibits phospholipase A2, blocks the release of arachidonic acid and biosynthesis of endoperekisy, PG, leukotrienes (contributing to the development of inflammation, allergies and other pathological processes).

Pharmacokinetics

With external use of the drug in therapeutic doses, transdermal absorption of the active substance into the blood is very small. The use of occlusive dressings, inflammation and skin diseases increase the transdermal absorption of betamethasone,which may lead to an increased risk of systemic side effects. Metabolized mainly in the liver. Excreted by the kidneys and with bile.

Use during pregnancy and lactation

Use of the drug Beloderm^® express, spray for external use, in pregnant women are allowed in cases where the intended benefit to the mother exceeds the risk to the fetus. In such cases, the use of the drug should be under the supervision of a physician, should be short-lived and, if possible, be limited to small areas of the skin.

If necessary, the use of the drug during lactation should stop breastfeeding.

Side effects

When used in therapeutic doses, the drug is usually well tolerated, side effects are absent or mild. As with the use of other corticosteroids, hypersensitivity reactions may develop (skin itching, burning, redness, dry skin, acne-like changes (steroid acne, perioral dermatitis, allergic contact dermatitis, hypopigmentation). With prolonged use, as well as with occlusive dressings, maceration skin, secondary infections, skin atrophy, hirsutism, hypertrichosis, telangiectasia, folliculitis, prickly heat, purpura. When applied to extensive body surfaces, systemic side effects of GCS (hyper glycemia, glucosuria, reversible inhibition of the function of the adrenal cortex, manifestation of Itsenko-Cushing syndrome).

Dosage and administration

Outwardly.

Beloderm^® Express, spray for external use, is sprayed onto the affected skin area in an amount necessary to cover the affected surface 2 times a day. The duration of treatment depends on the efficacy and tolerability of therapy and is usually no more than 4 weeks. During the year, may repeat therapy.In children and patients with lesions of the skin of the face, the course of treatment should not exceed 5 days. If clinical improvement does not occur, it is necessary to clarify the diagnosis.

Overdosage

Symptoms: acute overdose is unlikely, however, with excessive or prolonged use of the drug, especially in children, on large skin surfaces, applied to the skin with impaired integrity or when applied under an occlusive dressing, chronic overdose is possible, accompanied by signs of hypercortisolism (hyperglycemia, glycosuria, reversible suppression of the adrenal cortex, manifestation of Itsenko-Cushing syndrome).

Treatment: Gradual withdrawal of the drug and symptomatic treatment is recommended.

Special notes

Beloderm^® Express in the form of a spray is preferred for use in the acute form of dermatosis, including accompanied by exudation (formation of moist surfaces).

Long-term external use of the drug Beloderm is not recommended.^® express on the skin of the face, because development of rosacea, perioral dermatitis and acne is possible. The course of treatment should not exceed 5 days.

Beloderm^® Express should not be used in the eye area due to the likelihood of getting the drug on the mucous membrane, which may contribute to the development of cataracts, glaucoma, fungal infections of the eyes and exacerbation of herpes infection.

Some areas of the body, such as the armpits, inguinal folds, where there is a kind of natural occlusion, are more at risk of striae, therefore continuous use of the drug in these areas of the skin should be short-lived (no more than 4 weeks).

In the case of the development of a fungal or bacterial infection on the skin, additional use of an antibacterial or antifungal agent is necessary.

For children, the drug is prescribed only under strict indications and under medical supervision, because possible development of systemic side effects associated with betamethasone.

When using the drug in children, it is necessary to limit the total duration of treatment and exclude measures leading to increased resorption and absorption (warming, fixing and occlusive dressings, diapers).

When using the drug on large surfaces and / or under the occlusive dressing, suppression of the hypothalamic-pituitary-adrenal system and the development of symptoms of hypercorticism may occur, a decrease in growth hormone excretion, an increase in ICP may be observed.
