Indications and usage

- long-term treatment of coronary artery disease: prevention of strokes;

- chronic heart failure (as part of combination therapy with cardiac glycosides, ACE inhibitors and / or diuretics);

- secondary prevention of myocardial infarction and treatment with persistent angina (as part of combination therapy) for long-acting tablets.

Contraindications

- acute myocardial infarction with severe hypotension;

- acute vascular insufficiency (shock, vascular collapse);

- severe arterial hypotension (systolic pressure less than 90 mm Hg);

- angle-closure glaucoma with high intraocular pressure;

- traumatic brain injury;

- severe aortic and / or mitral stenosis;

- hypovolemia;

- hemorrhagic stroke;

- children's and teenage age up to 18 years (efficiency and safety of use are not established);

- simultaneous use of phosphodiesterase type 5 inhibitors (such as Sildenafil, vardenafil or tadalafil), since they potentiate the hypotensive effect of nitrates;

- hypersensitivity to nitrates.

Pregnancy and Breastfeeding

In pregnancy and lactation, the use of Cardiket is possible only when the intended benefit to the mother outweighs the possible risk to the fetus and / or the child.

Dosage and administration

Tablets of prolonged action 40 mg at the beginning of treatment is prescribed 1 pill 1 time / day. or 1/2 pill 2 times / day. With insufficient therapeutic effect, the dose can be increased to 1 pill 2 times / day. The second pill should be taken no later than 8 hours after the first.

Adverse reactions

Cardiovascular: at the beginning of treatment, a headache ("nitrate" headache) may develop, which usually disappears after a few days with the further use of the drug. Sometimes with the first use or with an increase in the dose of the drug, a pronounced decrease in blood pressure is observed, especially when the patient gets out of bed, which may be accompanied by tachycardia (paradoxical bradycardia is possible), lethargy, and dizziness and a feeling of weakness. In rare cases, with a pronounced decrease in blood pressure, there may be an increase in the symptoms of angina pectoris (a paradoxical increase in angina attacks). From the side of the central nervous system: stiffness, drowsiness, blurred vision, reduced ability to quickly mental and motor responses (especially at the beginning of treatment); rarely - brain ischemia.

Allergic reactions: skin rash.

Other: development of tolerance (including cross-over to other nitrates), exfoliative dermatitis, flushing of the skin.

Overdosage

Symptoms: pronounced decrease in blood pressure, collapse, syncope, headache, dizziness, feeling of heartbeat, visual disturbances, skin flushing, nausea, vomiting, diarrhea, methemoglobinemia (cyanosis, anoxia), hyperpnea, dyspnea, bradycardia, perspiration, skullcapitis, skullcapitis, dyspnea, bradycardia, perspiration, skullcapitis, skullcapitis, anoxia, hyperpnea, dyspnea; coma.

Treatment: gastric lavage; with methemoglobinemia, w / w in a 1% solution of methylene blue 1-2 mg / kg body weight. Symptomatic therapy (epinephrine and related compounds are ineffective).
