Dosage form
Tablets are dispersible white or white with a yellowish tinge, oblong, labeled "IOSA" and painted on one side and labeled "1000" on the other, with a sweet taste and the smell of strawberries.

Mechanism of action
Wilprafen Solutab - an antibiotic of the macrolide group. It has a bacteriostatic effect due to inhibition of protein synthesis by bacteria. When creating in the focus of inflammation of high concentrations has a bactericidal effect.
Highly active against intracellular microorganisms: Chlamydia trachomatis, Chlamydophila pneumoniae, Mycoplasma pneumoniae, Mycoplasma hominis, Ureaplasma urealyticum, Legionella pneumophila; against gram-positive aerobic bacteria: Staphylococcus aureus, Streptococcus pyogenes and Streptococcus pneumoniae, Corynebacterium diphtheriae; gram-negative aerobic bacteria: Neisseria meningitidis, Neisseria gonorrhoeae, Haemophilus influenzae, Bordetella pertussis; against some anaerobic bacteria: Peptococcus spp., Peptostreptococcus spp., Clostridium perfringens.
Slightly influences enterobacteria, therefore it does not change the natural bacterial flora of the gastrointestinal tract.
Effective with Erythromycin resistance. Resistance to josamycin develops less frequently than to other macrolide antibiotics.
Indications and usage
Acute and chronic infectious and inflammatory diseases caused by microorganisms sensitive to the drug: infections of the upper respiratory tract and ENT organs (including pharyngitis, tonsillitis, paratonsillitis, otitis media, sinusitis, laryngitis); diphtheria (in addition to treatment with diphtheria antitoxin); scarlet fever (with hypersensitivity to penicillin); lower respiratory tract infections (including acute bronchitis, exacerbation of chronic bronchitis, pneumonia, including those caused by atypical pathogens, whooping cough, psittacosis); infections of the oral cavity (including gingivitis and periodontal disease); eye infections (including blepharitis, dacryocystitis); infections of skin and soft tissues (including pyoderma, furunculosis, anthrax, erysipelas / with hypersensitivity to penicillin /, acne, lymphangitis, lymphadenitis); infections of the urinary tract and genital organs (including urethritis, prostatitis, gonorrhea, syphilis / with hypersensitivity to penicillin /, chlamydial, mycoplasma / including ureaplasma / and mixed infections).
Contraindications
Severe abnormal liver function;
Hypersensitivity to macrolide antibiotics.
Pregnancy and Breastfeeding
Allowed to use Wilprafen Solutab during pregnancy and lactation (breastfeeding) according to indications.
The WHO European Office recommends josamycin as the drug of choice for treating chlamydial infection in pregnant women.
Dosage and administration
The recommended daily intake for adults and adolescents over the age of 14 is 1-2 g.If necessary, the dose can be increased to 3 g / day. The daily dose should be divided into 2-3 doses.
The daily dose for children is established at the rate of 40-50 mg / kg of body weight in 2-3 doses.
For common and spherical acne, the recommended dose of the drug is 500 mg 2 times / day for the first 2-4 weeks, then 500 mg 1 time / day as maintenance therapy for 8 weeks.
Dispersible pills can be taken in 2 ways: swallow it whole with a glass of water, or dilute with water beforehand (at least 20 ml). Stir the resulting suspension well before taking it.
Usually the duration of treatment is determined by the doctor. In accordance with the WHO recommendations on the use of antibiotics, the duration of treatment of streptococcal infections should be at least 10 days.
When you skip taking Solpub Wilprafen, you must immediately take a dose of the drug. However, if it is time to take the next dose, you should not take the missed dose, you need to return to the normal treatment regimen. Do not take double dose. A break in treatment or premature discontinuation of the drug reduces the likelihood of treatment success.
Adverse reactions
Gastrointestinal: rarely - lack of appetite, nausea, heartburn, vomiting, dysbiosis, diarrhea; in some cases, a transient increase in liver transaminase activity, rarely accompanied by a violation of bile outflow and jaundice. In the case of the development of persistent severe diarrhea on the background of the drug intake, the possibility of pseudomembranous colitis should be considered.
Allergic reactions: in rare cases - skin reactions (urticaria).
From the organ of hearing: rarely, dose-dependent transient hearing loss.
Other: in some cases - candidiasis.
Special notes
Patients with renal insufficiency should be prescribed under the control of renal function.
When prescribing Wilprafen Solutab, consideration should be given to the possibility of cross-resistance to various macrolide antibiotics (for example, microorganisms that are resistant to treatment with antibiotics related to chemical structure may also be resistant to josamycin).
Drug interactions
Bacteriostatic antibiotics can reduce the bactericidal effect of other antibiotics, such as penicillins and cephalosporins (this combination should be avoided).
With simultaneous use of josamycin with Lincomycin may reduce the effectiveness of both drugs.
Some macrolide antibiotics slow down the elimination of xanthines (theophylline), which can lead to the development of the toxic action of the latter. Clinical and experimental studies have shown that josamycin has a less pronounced effect on theophylline excretion than other macrolide antibiotics.
With the simultaneous use of josamycin and antihistamine drugs containing terfenadine or astemizol, slowing the excretion of the latter is possible, which increases the risk of life-threatening arrhythmias.
There are separate reports of increased vasoconstriction with the simultaneous use of macrolide antibiotics and ergot alkaloids. Given this, while using josamycin and ergotamine, the patient’s condition should be monitored.
With simultaneous use of josamycin and cyclosporine, it is possible to increase the level of cyclosporine in the blood plasma and create its nephrotoxic concentration. Therefore, the combination of these drugs should regularly monitor the concentration of cyclosporine in the blood plasma.
With simultaneous use of josamycin and Digoxin may increase the level of the latter in the blood plasma.
In rare cases, during the treatment with macrolides, the contraceptive effect of hormonal contraceptives may not be sufficiently effective (it may be necessary to use non-hormonal methods of contraception).
Overdosage
So far, there are no data on the specific symptoms of an overdose of Solpub Wilprafen. In case of overdose, the occurrence and intensification of side effects on the part of the digestive system should be assumed.
