Mechanism of action

Tranquilizer. A drug from the group of benzodiazepine derivatives (an atypical benzodiazepine derivative). It has an anxiolytic effect, not accompanied by sedative, muscle relaxant, anticonvulsant action. It is a psycho-vegetative regulator, eliminates various forms of autonomic disorders. It has moderate stimulating activity, improves psychomotor and intellectual functions.
Due to the lack of muscle relaxant effect, the drug can be used in patients with myopathy and myasthenia.
Due to the atypical nature of the chemical structure, in contrast to the classical benzodiazepine derivatives, Grandaxin in therapeutic doses does not potentiate the effect of alcohol and practically does not cause the development of physical, psychological dependence and withdrawal syndrome.
Grandaxine refers to daytime tranquilizers.

Suction
After ingestion, it is rapidly and almost completely absorbed from the gastrointestinal tract. C_max in the blood plasma is reached within 2 hours, after which the plasma concentration decreases monoexponentially.
Distribution
Tofizopam not accumulates in the body.
Metabolism
Tofisopam metabolites do not possess pharmacological activity.
Removal
T_1/2 is 6-8 hours
It is mainly excreted with urine (60-80%) in the form of conjugates with glucuronic acid, and to a lesser extent (about 30%) with feces.

Indications and usage

- neurosis and neurosis-like states;
- states accompanied by emotional stress, vegetative disorders, moderately expressed fear, apathy, decreased activity, obsessive feelings;
- reactive depression with moderately pronounced psychopathological symptoms;
- post-traumatic stress disorder;
- mental adaptation disorder;
- climacteric syndrome (as an independent tool, as well as in combination with hormonal drugs);
- premenstrual tension syndrome;
- cardialgia (as monotherapy or in combination with other drugs);
- chronic alcoholism, alcohol withdrawal syndrome, delirium;
- myasthenia, myopathies, neurogenic muscular atrophies and other pathological conditions with secondary neurotic symptoms in cases when anxiolytics are contraindicated with a pronounced muscle relaxant effect.

Set individually, taking into account the patient's condition, the clinical form of the disease and individual sensitivity to the drug.
For adults appoint 50-100 mg (1-2 tab.) 1-3 times / day. The maximum daily dose of 300 mg.
Children over 14 years old appoint up to 200 mg / day.
To prevent sleep disorders, it is recommended to take the drug no later than 15-16 hours a day.
Elderly patients and patients with renal insufficiency the daily dose is reduced by about 2 times.
At alcohol withdrawal syndromefor prevention and treatment delirium a short course of therapy is shown: from several days to 3-6 weeks.

Adverse reactions

From the side of the central nervous system: psychomotor agitation, irritability, aggressiveness, sleep disturbances are possible.
Gastrointestinal: possible nausea, gastralgia.
Allergic reactions: possible pruritus, rash.

Contraindications

- psychosis and psychopathy, accompanied by pronounced psychomotor agitation, aggressiveness or deep depression;
- respiratory failure in the stage of decompensation;
- I trimester of pregnancy;
- lactation (breastfeeding);
- Hypersensitivity to the drug.

Pregnancy and breastfeeding

Grandaksin is contraindicated for use in the first trimester of pregnancy and lactation.

Special notes

Treatment of Grandaxine in patients with myasthenia should be carried out under strict constant medical supervision.
With affective-shock reactions within the framework of reactive psychosis, as well as with deep depression, active combination therapy with the use of antidepressants is indicated.
The appearance of increased excitability during the treatment with Grandaxin, dyspepsia in some cases can be eliminated by reducing the dose of the drug.
With the development of insomnia, the last drug intake should be done no later than 15 hours. The appearance of exanthema in the course of treatment with Grandaxinum requires discontinuation of the drug.
In the case of allergic reactions to the drug or serious sleep disorders, Grandaxine should be stopped.
Influence on ability to drive motor transport and control mechanisms
Grandaxine does not cause a significant decrease in the ability to concentrate.
The question of the possibility of driving vehicles and driving mechanisms should be solved only after evaluating the patient's individual response to the drug.

Overdosage

Symptoms: drowsiness, slight respiratory depression, in severe cases - coma.
Treatment: carrying out activities aimed at restoring respiratory function and cardiovascular activity. Recommended gastric lavage, taking activated charcoal. In case of emergency it is possible to use a specific benzodiazepine antagonist - flumazenil.

Pharmacokinetic interaction
With the simultaneous use of Grandaxine and Metoclopramide or other drugs that enhance gastrointestinal peristalsis, the absorption of tofisopam in the intestine is accelerated as a result of a more rapid evacuation of the contents of the stomach into the intestine (Grandaxine action occurs faster).

The drug should be stored at a temperature of 15 ° to 25 ° C. Shelf life - 5 years.
