Mechanism of action

Etaperazine - an antipsychotic (neuroleptic), piperazine derivative of phenothiazine. It is believed that the antipsychotic effect of phenothiazines is due to the blockade of postsynaptic dopamine receptors in the mesolimbic structures of the brain. Perphenazine has a strong antiemetic effect, the central mechanism of which is associated with the inhibition or blockade of dopamine D₂-receptors in the chemoreceptor trigger zone of the cerebellum, and peripheral - with a blockage of the vagus nerve in the gastrointestinal tract. It has alpha-adrenergic blocking activity. Anticholinergic activity and sedation can manifest from low to moderate degrees of intensity, the hypotensive effect is mild. It has a pronounced extrapyramidal effect. The antiemetic effect may be enhanced by anticholinergic and sedative properties. It has a muscle-relaxing effect.

Treatment of psychotic disorders, especially with hyperactivity and agitation, schizophrenia; neurosis, accompanied by fear, tension. Treatment of nausea and vomiting of various etiologies. Pruritus

Contraindications

Cirrhosis, hepatitis, hemolytic jaundice, nephritis, hematopoietic disorders, myxedema, progressive systemic diseases of the brain and spinal cord, decompensated heart disease, thromboembolic diseases, late stages of bronchiectasis, pregnancy, lactation, hypersensitivity to perphenazine.

Dosage and administration

For adults and children over 12 years of age when administered orally, the daily dose is 4-80 mg. In the chronic course of the disease and in resistant cases, the daily dose may be increased to 150-400 mg. The frequency of treatment and the duration of the course of treatment are set individually.

Adverse reactions

Drug Interactions

With simultaneous use with drugs that cause extrapyramidal reactions, it is possible to increase the frequency and severity of extrapyramidal disorders.

At simultaneous use with the drugs causing arterial hypotension, the expressed orthostatic hypotension is possible.

With simultaneous use with agents with anticholinergic effects, it is possible to enhance their anticholinergic effects, while the antipsychotic effect of a neuroleptic may be reduced.

With simultaneous use with tricyclic antidepressants, maprotiline, MAO inhibitors increases the risk of developing NNS.

With the simultaneous use of antacids, anti-parkinsonian drugs, lithium salts, the absorption of phenothiazines is impaired.

With simultaneous use may reduce the action of amphetamines, levodopa, clonidine, guanetidina, epinephrine.

With simultaneous use with Fluoxetine, it is possible to develop extrapyramidal symptoms and dystonia.

With simultaneous use may weaken the vasoconstrictor effect of ephedrine.
