Mechanism of action
Fluconazole is an antifungal agent from the group of triazole derivatives. Inhibits the synthesis of ergosterol, violating the permeability of the cell wall. Active against Candida spp., Microsporum spp., Cryptococcus neoformans, Trichophyton spp.
Indications and usage
Cryptococcosis (including cryptococcal meningitis), systemic candidiasis, mucosal candidiasis, vaginal candidiasis, prevention of fungal infections in patients with reduced immunity (including AIDS patients).
Mycoses of the skin, including mycoses of the feet, body, groin; pityriasis versicolor; onychomycosis.
Deep endemic mycoses, including coccidioidomycosis, paracoccidioidomycosis, sporotrichosis and histoplasmosis in patients with normal immunity.
Contraindications
Pregnancy, age up to 1 year, hypersensitivity to fluconazole and triazole compounds.
Dosage and administration
Dosage regimen is individual. Depending on the indications, the daily dose is 50-400 mg, the frequency of use is 1 time / day. In case of impaired renal function, the dose of fluconazole should be reduced.
Adverse reactions
Gastrointestinal: nausea, abdominal pain, diarrhea, flatulence.
From the side of the central nervous system: headache, dizziness.
Allergic reactions: skin rash, Anaphylactic reactions.
Cautions
Fluconazole should be used with caution in patients with severe impaired hepatic or renal function.
Therapy can be started before the results of seeding and other laboratory tests. However, anti-infective therapy needs to be adjusted accordingly when the results of these studies become known.
Drug Interactions
With simultaneous use with coumarin anticoagulants, an increase in prothrombin time has been observed.
Fluconazole increases the half-life of oral hypoglycemic agents - sulfonylurea derivatives (chlorpropanide, glibenclamide, glipizide, tolbutamide) in healthy people. The combined use of fluconazole and oral hypoglycemic agents in patients with diabetes is allowed, but the physician should be aware of the possibility of hypoglycemia.
With simultaneous use with rifampicin, shortening of the half-life of fluconazole is noted.
In a dry, dark place at a temperature not higher than 25 ° C.
