Active ingredient and dosage form

Gordox - injection solution:

1 ampoule contains aprotinin, KIE 10,000; 25 pieces per pack.

Mechanism of action

Antiprotease, antifibrinolytic. Gordox inactivates proteinases (plasmin, kininogenases, trypsin, chymotrypsin, etc.) in plasma and tissues; lowers blood fibrinolytic activity.

Indications and usage

Acute and chronic pancreatitis, pancreatic necrosis, prevention of enzymatic autolysis of the pancreas during operations on it and nearby organs; hyperfibrinolysis; acute postoperative parotitis; severe forms of various types of shock; angioedema; extensive and deep tissue damage.

Contraindications

Hypersensitivity, DIC syndrome.

Side effects

Allergic reactions.

Dosage and administration

Gordox is injected IV slowly, exclusively in the patient’s lying position. Test dose: At least 10 minutes prior to the initial dose of the IV, a test dose of 1 ml (10,000 KIE Aprotinin) is administered to determine drug. For therapeutic purposes: the initial dose is 50000 KIE (maximum speed - 5 ml / min), then in / in the drip at 50,000 KIE / h.

Adverse reactions

On the CNS side: psychotic reactions, hallucinations, confusion.

Allergic reactions: urticaria, itching, rhinitis, conjunctivitis, bronchospasm, anaphylaxis, anaphylactoid reactions (rash, pruritus, shortness of breath, nausea, rapid heart rate - these symptoms may progress to anaphylactic shock with symptoms of circulatory insufficiency in some cases fatal). With repeated administration of the drug, the frequency of Anaphylactic reactions is less than 0.5%. Even with satisfactory tolerability of the second dose, further administration of aprotinin can cause severe anaphylaxis, the risk of which continues to increase with repeated doses. In some cases, an anaphylactoid reaction is observed already after the first dose. In the case of hypersensitivity reactions during infusion, the administration of the drug should be immediately stopped, if necessary, to carry out standard emergency treatment measures (for example, the administration of epinephrine, GCS, rehydration of the body). During surgical operations on the heart and the introduction of aprotinin in high doses, it is possible (<1%) to temporarily increase the level of creatinine, in very rare cases accompanied by clinically significant symptoms.

On the side of the cardiovascular system: an increase in the incidence of myocardial infarction with repeated by-pass operations on the coronary vessels as compared with the control groups (the mortality rates are the same); decrease in blood pressure, tachycardia.

Special notes

If symptoms of a side effect of the drug occur during administration, you should immediately stop administering Gordox.

Storage conditions

The drug should be stored out of reach of children at a temperature not exceeding 25 ° C. Shelf life - 5 years.
