Beloderm ointment
Pharmachologic effect:
Beloderm ointment contains betamethasone dipropionate as an active component - a synthetically prepared adrenocorticosteroid (an analogue of prednisolone).
Betamethasone dipropionate has a significantly more pronounced corticosteroid activity and a slight mineralocorticoid effect.
Beloderm has a pronounced anti-inflammatory, immunosuppressive and anti-allergic effect. When applying the ointment, there is a decrease in the production and release of histamine, prostaglandins and lysosomal enzymes. In addition, glucocorticosteroids have a specific effect on cell migration to the site of inflammation, which helps to reduce extravasation of the plasma and the development of edema.
Beloderm reduces the negative impact of antigen-antibody complexes and reduces the effect of target cell lymphokines, as well as of macrophages, in addition, the drug prevents access of sensitized T-lymphocytes and macrophages to target cells. When applying Beloderm ointment, a slowdown in the development of allergic dermatitis is noted.
The absorption of the active ingredient into the systemic circulation significantly varies depending on the integrity of the skin, the presence of inflammation and the condition of the skin (when applied to the damaged epidermis, the absorption increases).Betamethasone absorption on intact skin is negligible.
About 64% of betamethasone in plasma binds to proteins, metabolism takes place in the liver.
Excretion - with bile and urine (no more than 5%).
Indications for use:
Beloderm is used in the treatment of dermatoses requiring glucocorticoid therapy:
- Atopic and coin-like eczema.
- Solar, seborrheic, contact, exfoliative, radiation and intertriginous dermatitis.
- Neurodermatitis.
- Psoriasis.
- Itching anogenital area.
- Staz-dermatitis.
Mode of application:
Only external use of ointment is allowed. Ointment is easily rubbed into cleansed, dry skin up to 3 times a day. It is recommended to apply a small amount of ointment, sufficient to create a thin ball of the drug on the treated area.
In most cases, it is enough to use the drug Beloderm 1-2 times a day. If the next application was missed, you should use the ointment as soon as the patient remembers. If the application was missed and the next application is already necessary, the dose cannot be doubled.
Occlusive dressings enhance the systemic effect of Beloderm ointment, therefore it is not recommended to use them.
Avoid getting betamethasone in the eyes.
During treatment, it is necessary to control the function of the hypothalamic-pituitary-adrenal system, with signs of oppression, such Beloderm is canceled.
Side effects:
When applied externally, Betamethasone can cause collagen reduction and other changes in the skin, resulting in stretch marks, skin atrophy, ecchymosis, folliculitis, telangiectasia and hypertrichosis.
With long-term treatment there is a risk of rash, hyperpigmentation, itching, depigmentation of the skin and hair. In addition, with prolonged use of betamethasone may decrease the function of the sweat glands.
Immunosuppression may cause secondary infection of the skin.
Systemic side effects were practically not observed with the standard treatment regimen; however, their risk increased sharply with overdose and prolonged treatment with large doses of betamethasone.
When skin reactions occur, Beloderm ointment is immediately discontinued.
Contraindications:
Absolute contraindications:
- Intolerance to betamethasone and excipients.
- Tuberculosis of the skin.
- Phlebeurysm.
- Rosacea.
- Skin reactions after vaccination.
- Perioral dermatitis.
- Other infectious skin diseases associated with bacteria and fungi (without adequate fungicidal and antibacterial therapy).
It is strictly prohibited to apply ointment to the periorbital area.
Relative contraindications:
- Facial skin lesions requiring the use of Beloderm ointment (ointment can be applied on the face for no more than 1 week).
- Insufficiency of liver function (only short-term use of low doses of betamethasone without the use of dressings is allowed).
- With caution, ointment is applied to the folds of the body, where there is a natural occlusion (the risk of systemic action increases).
In pediatrics, Beloderm is used especially carefully, taking into account the peculiarities of percutaneous absorption (in children, the absorption of betamethasone is higher), as well as the ratio of body area to the treated area. The use of ointments for diapers is strictly forbidden.
Pregnancy:
Pregnant Beloderm prescribed only for health reasons and under the strict supervision of a physician and the level of the drug in the plasma. The effect of betamethasone topical use on pregnant women and fetal development has not been studied.
If you need betamethasone therapy, breastfeeding should be stopped.
Interaction with other drugs:
Clinical interactions have not been studied.
Overdose:
Systemic overdose is possible when applying a large amount of the drug, as well as the use of ointment on a large area and the use of occlusive coatings. Chronic overdose is possible when using Beloderm ointment for more than 3 weeks in a row.
Overdose is accompanied by inhibition of the function of the hypothalamus-pituitary-adrenal system and concomitant growth retardation and intracranial hypertension (in children), as well as impaired glucose metabolism (glycosuria and hyperglycemia) and Cushing's syndrome.
In children, inhibition of the hypothalamic-pituitary-adrenal system function is accompanied by a slowdown in growth and weight gain, a decrease in serum and urine cortisol, as well as a lack of response to stimulation of ACTH.Symptoms of intracranial hypertension are tension, pulsation and swelling of the crown, as well as headaches.
In case of overdose should:
- Immediately cancel Beloderm.
- To carry out measures to restore the water-salt balance.
- Prescribe symptomatic therapy.
In chronic intoxication, betamethasone may be gradually discontinued.
