Mechanism of action

A broad-spectrum antibiotic that is active against gram-positive and gram-negative microorganisms (including strains that produce beta-lactamase). Bactericidal effect. Clavulanic acid, which is part of the preparation, ensures the resistance of Amoxicillin to the effects of beta-lactamase, expanding its spectrum of action.
Augmentin active against aerobic gram-positive bacteria: Staphylococcus aureus (including strains that produce beta-lactamase), Staphylococcus epidermidis (including strains that produce beta-lactamase), Streptococcus pyogenes, Streptococcus anthracis, Streptococcus pneumoniae, Streptococcus viridans, and, as a company, a company, a company, a company, a company, a company, a book, a campaign, a book, a beta, a lactamase, a beta-lactamase producing beta-lactamase; anaerobic gram-positive bacteria: Clostridium spp., Peptococcus spp., Peptostreptococcus spp.; aerobic gram-negative bacteria: Escherichia coli (including strains producing beta-lactamase), Proteus mirabilis (including strains producing beta-lactamase), Proteus vulgaris (including strains producing beta-lactamase), Klebsiella spp. (including beta-lactamase producing strains), Salmonella spp. (including beta-lactamase producing strains), Shigella spp. (including strains producing beta-lactamase), Bordetella pertussis, Yersinia enterocolitica (such as strains producing beta-lactamase), Gardnerella vaginalis, Brucella spp., Neisseria meningitidis, Neisseria gonorrhoeae (including strains (including items, in the case, and in the case, and in the case, in the case of strains, including strains, Neisseria meningitidis, Neisseria gonorrhoeae (including strains, Neisseria meningitidis, Neisseria gonorrhoeae) Haemophilus influenzae (including strains producing beta-lactamase), Haemophilus ducreyi (including strains producing beta-lactamase), Pasteurella multocida, Campylobacter jejuni, Vibrio cholerae; anaerobic gram-negative bacteria: Bacteroides spp., Including Bacteroides fragilis (including strains producing beta-lactamase).

Suction
After taking the drug, both active substances are rapidly absorbed from the gastrointestinal tract. After oral administration Augmentina dose of 375 mg and 625 mg Cmax Amoxicillin in the blood plasma is 3.7 mg / l and 6.5 mg / l, respectively, clavulanic acid, 2.2 mg / l and 2.8 mg / l, respectively. After the on / in the dose of 1.2 g and 600 mg Cmax Amoxicillin in the blood plasma is 105.4 mcg / ml and 32.2 mcg / ml, respectively, clavulanic acid, 28.5 mcg / ml and 10.5 mcg / ml, respectively.
Distribution
After intravenous Augmentation, amoxicillin and clavulanic acid in therapeutic concentrations can be detected in tissues and interstitial fluids. Therapeutic concentrations of both active substances are also determined in the gallbladder, in the skin, in the tissues of the abdominal cavity, in adipose and muscle tissues, in synovial and peritoneal fluids, in bile and purulent discharge. Plasma protein binding is 25% for clavulanic acid and 18% for amoxicillin.
Removal
The main route of elimination is renal. After a single oral administration of one tablet, 375 mg each (250 mg of amoxicillin and 125 mg of clavulanic acid) or 625 mg (500 mg and 125 mg, respectively) of approximately 60-70% of amoxicillin and 40-60% of clavulanic acid are excreted with urine unchanged.
Average t1/2 Amoxicillin and clavulanic acid after administration of Augmentin at a dose of 375 mg and 625 mg is 1 h and 1.3 h for amoxicillin, 1.2 h and 0.8 h for clavulanic acid, respectively.
Average t1/2 amoxicillin and clavulanic acid after IV injection of Augmentin at a dose of 1.2 g and 600 mg is 0.9 h and 1.07 h for amoxicillin, 0.9 h and 1.12 h for clavulanic acid, respectively.

Indications and usage

Infectious and inflammatory diseases caused by microorganisms sensitive to the drug:
- infections of the upper respiratory tract and ENT organs (including otitis media, sinusitis, tonsillitis, pharyngitis);
- infections of the lower respiratory tract (including acute and chronic bronchitis, lobar pneumonia, bronchopneumonia, empyema, lung abscess);
- bacterial infections of the skin and soft tissues (including abscess, cellulitis, wound infection);
- infections of the urinary tract (including cystitis, urethritis, pyelonephritis, salpingitis, salpingoophoritis, tubo-ovarian abscess, endometritis, bacterial vaginitis, septic abortion, postpartum sepsis, pelvioperitonitis, shankroid, gonorrhea);
- infections of bones and joints (including osteomyelitis);
- sepsis;
- peritonitis;
- postoperative infections.
Prevention of infectious and inflammatory complications during surgical operations on the gastrointestinal tract, pelvic organs, on the head and neck, heart, kidneys, biliary tract and during reconstructive reconstructive operations (intravenous administration).

Set individually depending on the severity, localization of infection and sensitivity of the pathogen. Adults and children over 12 years old at mild to moderate infections appoint 1 tab. 375 mg 3 times / day. At severe infections single dose is 1 tab. on 625 mg or 2 tab. 375 mg 3 times / day or 1 tab. 1 g 2 times / day. It is also possible in / in the introduction of the drug in a single dose of 1.2 g every 8 hours. If necessary, the drug can be administered every 6 hours. The maximum single dose is 1.2 g, the maximum allowable daily dose for intravenous administration is 7.2 g.
Children under the age of 12 the drug is prescribed in the form of a syrup. A single dose depends on age and is:

AgeSingle dose of the drug in the form of a syrup (156 mg / 5 ml)
7-12 years old10 ml
2-7 years5 ml
from 9 months to 2 years2.5 ml

The multiplicity of appointments - 3 times / day. With more severe infections, these doses can be doubled. For children under 9 months dose for oral administration is not installed.
In / in a single dose for children aged from 3 months to 12 years makes 30 mg / kg of body weight; the drug is administered every 8 hours, with severe infections - every 6 hours. Children up to 3 months administer the drug in a single dose at the rate of 30 mg / kg body weight: premature and children in the perinatal period - every 12 hours, then every 8 hours. The treatment should not be continued for more than 14 days without revising the clinical situation.
For prevention of postoperative complications with operations lasting less than 1 hour, Augmentin at a dose of 1.2 g is administered IV during induction anesthesia. For longer operations, 1.2 g is prescribed up to 4 times within 24 hours. With a higher risk of infection, this dosing regimen can be continued for several days.
Adult patients with impaired renal excretory function of moderate or severe Correction of Augmentin dosing regimen required. At QC more than 30 ml / min no change in dosage regimen is required. At CC 10-30 ml / min The initial dose of the drug is 1.2 g IV, then 600 mg every 12 hours. When QC less than 10 ml / min The initial dose of the drug is 1.2 g, then 600 mg every 24 hours. Augmentin is eliminated during dialysis. In the case of use of the drug in patients on dialysis, an additional intravenous administration of the drug is prescribed at a dose of 600 mg at the time and 600 mg at the end of the dialysis procedure.
