Composition
1 capsule contains:
active ingredient: diacerein 50.0 mg;
excipients: corn starch 30.0 mg, lactose monohydrate 188.95 mg, colloidal silicon dioxide (aerosil) 1.5 mg, povidone K-30 2.2 mg, crospovidone (type A) 24.0 mg, sodium lauryl sulfate 0, 35 mg, Magnesium stearate 3.0 mg;
hard gelatin capsules: case and cap of the capsule - titanium dioxide 1.3333%, dye quinoline yellow 0.9197%, dye sun-setting yellow 0.0044%, gelatin up to 100%.
pharmachologic effect
Diacerein is an anthraquinoline derivative, a diacetylated rhein derivative. It is metabolized to the active metabolite of Rein, inhibits the activity of interleukin-1, which plays an important role in the development of inflammation and degradation of cartilage in osteoarthritis.
Diacerein inhibits the action of other cytokines, including interleukin-6, tumor necrosis factor-a. The action develops in 2-4 weeks.
Diacerein has analgesic and anti-inflammatory activity when taken orally.
Indications
Primary and secondary osteoarthritis.
Contraindications
Children and adolescents up to 18 years; hypersensitivity to diacerein; inflammatory bowel disease (ulcerative colitis, Crohn's disease); intestinal obstruction; abdominal pain of obscure origin; patients with liver disease (includingin history); severe renal failure (CC less than 30 ml / min); lactase deficiency, lactose intolerance, glucose-galactose malabsorption; pregnancy and breastfeeding period.
Side effects
On the part of the digestive system: diarrhea, nausea, vomiting, abdominal pain, hepatitis.
Allergic reactions: urticaria, fever, bronchospasm, angioedema, anaphylactic shock.
Other: malaise, intense urine staining (depending on pH) from yellow to brown (no clinical significance).
Interaction
Diacerein is not recommended to be taken together with drugs that increase the volume of intestinal contents (for example, a large amount of fiber).
Antacids reduce the absorption of diacerein.
When taken simultaneously with antibiotics or chemotherapeutic drugs that affect the intestinal microflora, the incidence of adverse reactions from the intestine may increase.
Drugs containing aluminum hydroxide and / or magnesium hydroxide, reduce the bioavailability of diacerein.
Simultaneous use with laxatives is not recommended.
Inside, after eating, swallowing whole with water, 50 mg (1 capsule) 2 times a day (in the morning and in the evening).
The treatment is carried out continuously for a long time or courses of at least 4 months.
Since diacerein during the first two weeks may cause an acceleration of transit in the intestine, it is recommended to start treatment with 1 capsule per day (in the evening, with meals) for four weeks.Then, after consulting a doctor, the dose may be increased to 100 mg (2 capsules) per day.
Overdose
Symptoms: diarrhea, weakness.
Treatment: symptomatic. In severe diarrhea, it is necessary to replenish water and electrolyte balance.
Use during pregnancy
Use during pregnancy and during breastfeeding is contraindicated.
Special instructions
The duration of treatment may be long. Before the onset of the therapeutic effect, diacerein can be taken simultaneously with analgesic drugs (including nonsteroidal anti-inflammatory drugs).
During treatment, periodic monitoring of blood parameters, functional liver and urine samples is necessary.
When kidney function deteriorates, the doctor must decide to reduce the dose of the drug or cancel it.
Staining urine can make it difficult to interpret the results of a urine test (for example, glucose content).
When a patient has diarrhea, drug treatment should be discontinued.
