Mechanism of action
H inhibitor+-K+-ATP-ase. Inhibits the activity of H+-K+-ATP-ase in the parietal cells of the stomach and thereby blocks the final stage of the secretion of hydrochloric acid. This leads to a decrease in basal and stimulated secretion, regardless of the nature of the stimulus. As a consequence of the decrease in acid secretion, the drug reduces or normalizes the effect of acid on the esophagus in patients with reflux esophagitis.
Omeprazole has a bactericidal effect on Helicobacter pylori. Eradication of H. pylori with simultaneous use of Omeprazole and antibiotics allows you to quickly arrest the symptoms of the disease, achieve a high degree of healing of the damaged mucous and persistent long-term remission and reduce the likelihood of bleeding from the gastrointestinal tract.
Peptic ulcer and duodenal ulcer in the acute phase (including associated with Helicobacter pylori), reflux esophagitis, Zollinger-Ellison syndrome, erosive-ulcerative lesions of the stomach and duodenum, associated with the intake of NSAIDs.
Contraindications
Chronic liver disease (including a history), hypersensitivity to omeprazole.
Pregnancy and Breastfeeding
Due to the lack of clinical experience, omeprazole is not recommended for use during pregnancy. If necessary, use during lactation should decide on the termination of breastfeeding.
Dosage and administration
Dosage regimen is individual. For ingestion, a single dose is 20–40 mg. Daily dose - 20–80 mg; frequency of use - 1-2 times / day. Duration of treatment is 2–8 weeks.
Adverse reactions
Gastrointestinal: rarely - nausea, diarrhea, constipation, abdominal pain, flatulence, dyspeptic disorders.
From the side of the central nervous system: rarely - headache, dizziness, weakness.
Hemic and lymphatic: in some cases - anemia, eosinopenia, neutropenia, thrombocytopenia.
Urogenital: in some cases - hematuria, proteinuria.
Musculoskeletal system: in some cases - arthralgia, muscle weakness, myalgia.
Allergic reactions: rarely a skin rash.
Before starting therapy, it is necessary to exclude the possibility of the presence of a malignant process (especially in case of a stomach ulcer), since Omeprazole treatment may mask the symptoms and delay the proper diagnosis.
Against the background of the use of omeprazole may be a distortion of the results of laboratory studies of liver function and indicators of the concentration of gastrin in the blood plasma.
Due to the lack of experience with clinical use of omeprazole is not recommended for use in children.
Drug Interactions
While taking omeprazole may decrease the absorption of Ketoconazole.
Omeprazole slows excretion of diazepam, Warfarin, phenytoin, as well as other drugs that are metabolized in the liver with the participation of cytochrome P450 2C19.
With long-term simultaneous use of omeprazole and Clarithromycin, their plasma concentrations increase.
With simultaneous use of omeprazole with antacids drug interaction is not marked.
Overdosage
Symptoms: blurred vision, drowsiness, agitation, flushing, headache, increased sweating, dry mouth, nausea, tachycardia.
Treatment: conduct symptomatic and supportive therapy. There is no specific antidote.
The drug should be stored in a dry, dark place at room temperature. Keep out of the reach of children.
