Composition and release form
Himopsin - lyophilisate for preparation of solution for topical and external use:
1 bottle contains as active substances - a mixture of trypsin and chymotrypsin - 50 mg; per pack of 10 bottles.
Pharmachologic effect:
Himopsin is a proteolytic combination agent of protein nature. Prepared from the pancreas of cattle. When local and external use splits necrotic tissue and fibrinous formations; thins viscous secretion, exudate, blood clots. Himopsin hydrolyzes proteins and peptones to form low molecular weight peptides, cleaves bonds formed by aromatic amino acid residues (tyrosine, troptophan, phenylalanine, methionine).
Indications
Suppurative bronchopulmonary diseases (bronchiectasis, lung abscess, atelectasis, exudative pleurisy). Diseases of ENT organs: purulent sinusitis, purulent otitis media (acute, subacute, chronic), eustachitis with viscous exudate, after a tracheostomy to facilitate the removal of thick viscous exudate, chronic rhinitis. Burns, bedsores, trophic ulcers, purulent wounds. In ophthalmology: corneal diseases (ulcer, herpetic keratitis); burns of the mucous membrane of the eye, obstruction of the lacrimal ducts, flaccid healing of the eyelid skin wounds.
Contraindications
Hypersensitivity, malignant neoplasms, destructive forms of pulmonary tuberculosis, CHF II-III century, pulmonary emphysema with respiratory failure. Himopsin cannot be entered into the bleeding cavities, to apply on ulcerated surfaces of malignant tumors. With caution. Empyema pleura tuberculosis etiology (resorption of exudate may contribute to the development of bronchopleural fistula).
Side effects
Allergic reactions, fever to subfebrile, tachycardia. After inhalation administration of Himopsin: irritation of the mucous membrane of the upper respiratory tract, hoarseness. In ophthalmology: irritation and swelling of the conjunctiva (in this case, it is recommended to reduce the concentration of the solution used).
Dosage and administration
Himopsin apply topically, externally. For the treatment of purulent wounds and bedsores, sterile wipes moistened with a solution: 25-50 mg of Himopsin in 10-50 ml of a 0.25% solution of procaine are applied for a period of 2 to 24 hours (depending on the thickness of the necrotic masses) on the wound surface. Bandages change every 3-5 days. When thermal burns III Art. a thin layer of the preparation is applied to the eschar at the rate of 1 g per 100 sq. cm of the wound surface, covered with a dressing moistened with 0.9% NaCl solution or 0.25% procaine solution; impose a water resistant dressing. Change dressings produced in a day. During each dressing change, easily detachable areas of necrotic tissue are removed.For wounds covered with a thick scab, it is cut before applying the bandage. In case of suppurative bronchopulmonary diseases, inhalations of 25-30 mg of Himopsin diluted in 5 ml of distilled water are used 1-3 times a day. The solution can also be administered through a bronchoscope, endotracheal probe, tracheostomy. Antibiotics, bronchodilating drugs can be added to the solution. For the treatment of ulcers, corneal burns and keratitis, they are used in the form of eye baths in a solution of 1: 500 daily, for 2-3 days or instilled 2 drops of 0.25% solution, 4 times a day, for 1-2 days. Obstruction of the lacrimal ducts and skin wounds of the eyelids: the lacrimal ducts are washed or irrigated with wounds with a 1% solution. Solutions are prepared ex tempore. In chronic rhinitis, instillation or irrigation of the solution (5 mg in 5 ml of 0.9% NaCl solution) of Himopsin nasal cavity 2-3 times a day is instilled. In chronic purulent otitis, complicated by cholesteatoma, after washing the cavity with a 0.9% NaCl solution, a 0.5% solution of the drug (diluted with a 0.9% NaCl solution) is instilled 2-3 times a day for 20-30 minutes.
