Mechanism of action
Aprovel is an antihypertensive drug, a specific angiotensin II receptor antagonist (type AT1). Eliminates the vasoconstrictor effect of angiotensin II and reduces the concentration of aldosterone in the blood plasma. Aprovel blocks all physiologically significant effects of angiotensin II, realized through the AT1 receptor, regardless of the source or route of synthesis of angiotensin II. The specific antagonistic effect on angiotensin II receptors (AT1) leads to an increase in plasma renin and angiotensin II concentrations and to a decrease in plasma aldosterone concentrations.
When applying the recommended doses of Aprovel, the concentration of potassium ion in the serum does not change significantly. Irbesartan does not inhibit kininase II, through which the formation of angiotensin II and the destruction of bradykinin to inactive metabolites. To show its effect, irbesartan does not require metabolic activation.
Indications and usage
Arterial hypertension; treatment of nephropathy in patients with hypertension and type 2 diabetes mellitus (as part of combination antihypertensive therapy).
Contraindications
- hereditary intolerance to galactose, lactase deficiency or impaired absorption of glucose and galactose;
- pregnancy;
- lactation period;
- children's and teenage age up to 18 years (efficiency and safety are not established);
- hypersensitivity to the drug.
Dosage and administration
Aprovel should be taken orally, the pill is swallowed whole, washed down with water. The initial and maintenance dose is 150 mg 1 time / day. regardless of the meal.
Adverse reactions
From the side of the central nervous system: often - dizziness.
Since the cardiovascular system: sometimes - tachycardia, hyperemia of the skin.
On the part of the respiratory system: sometimes - cough.
On the part of the digestive system: often - nausea, vomiting; sometimes - diarrhea, dyspepsia, heartburn.
On the part of the reproductive system: sometimes - sexual dysfunction.
On the part of the organism as a whole: often - fatigue; sometimes - pain in the chest.
Special notes
With dehydration and / or deficiency of sodium ions (as a result of intensive treatment with diuretics, diarrhea or vomiting, restriction of salt intake from food), as well as in patients on hemodialysis, clinically significant hypotension may develop, especially after taking the first dose of the drug. These pathological conditions must be corrected before starting to use Aprovel.
Storage conditions
The drug should be stored in a dry, dark place at a temperature not higher than 25 ° C.
