Mechanism of action

Emla is a combined preparation for local anesthesia. Lidocaine and prilocaine - amide-type local anesthetics, blocking voltage-dependent Na + channels, prevent the generation of impulses in the terminations of sensory nerves and conduction of impulses along nerve fibers. Suppress holding not only pain, but also impulses of other modalities of action. After application to the skin, the maximum severity of anesthesia develops after 60 minutes and lasts 30 minutes.

Surface anesthesia of the skin and mucous membranes: before performing surface surgical manipulations on the skin, taking skin grafts; before inserting a needle for blood collection or catheterization of blood vessels; before performing superficial surgical procedures or infiltrative anesthesia on the mucous membranes of the genital organs; before mechanical cleaning and excision of trophic ulcers on the lower limbs; before skin flap taking in combustiology and surgical practice; for tattoo removal.

Contraindications

Hypersensitivity to local anesthetics amide structure; methemoglobinemia (congenital or idiopathic), premature babies and children up to 12 months against the background of taking methemoglobin-inducing drugs; anesthesia of the skin before the introduction of live vaccines.

Dosage and administration

EMLA cream is applied under the occlusive dressing (available in the package) at least 1 hour before the procedure begins, the time of application of the drug is recorded on the dressing, and after removing the dressing, wipe the rest of the cream with ethanol. For anesthesia of the skin: adults (for carrying out "small" procedures) apply 1.5 g / 10 sq. Cm for 1-5 hours, on large surfaces - 1.5-2 g / 10 sq. Cm for 2-5 hours; children from 3 to 11 months - up to 1 g / 20 sq. cm. Children over 1 year old - 1 g / 10 sq. Cm. Anesthesia of the genital organs - 5-10 g of cream for 5-10 minutes (without a tight dressing). In atopic dermatitis, the duration of the application should not exceed 15-30 minutes. The plaster is fixed on the previously cleaned and dried skin area, recording the time of application on its surface. The minimum duration of application is 60 minutes, the maximum - 5 hours; at the same time it is possible to fix several patches. Children up to 3 months - 1 hour and not more than 1 patch, children 3-11 months - 1-4 hours, no more than 2 patches, 1-5 years - 1-5 hours, the maximum recommended dose is 10 patches, 6-11 years - 1-5 hours, no more than 20 patches. After using the patch, the surface of the skin is treated with an antiseptic.

Adverse reactions

Pallor, hyperemia, swelling, burning, itching in the area of ​​application; allergic reactions (including anaphylactic shock).

Cautions

After applying the cream from the tube on the ulcerated skin of the lower extremities, the tube with the remains of the ointment should be discarded. Do not apply on the mucous membrane and open wounds (with the exception of trophic ulcers of the lower extremities). Care must be taken when applying around the eyes (corneal irritation is possible). Penetrates into breast milk in concentrations that are safe for the baby.

Overdosage

Symptoms: CNS agitation, convulsions, coma, methemoglobinemia (caused by prilocain).

Treatment: pulmonary ventilation, maintenance of vital functions, symptomatic therapy; with methemoglobinemia - intravenously / slowly a solution of methylene blue, with convulsions - anticonvulsant therapy.

Store at temperatures below 30 ° C, out of the reach of children. Do not freeze.
