Active ingredient and dosage form

Vaginal suppositories:
1 suppository contains Metronidazole 500 mg and miconazole nitrate 100 mg;
Other Ingredients: Witepsol 1.9 g / 1 supp.
14 pcs. in strips of plastic complete with disposable fingertips in a cardboard bundle.

Mechanism of action

Combined preparation for intravaginal use with antifungal, antiprotozoal and antibacterial action.
Miconazole nitrate active against pathogenic fungi, especially Candida albicans, as well as some gram-positive bacteria.
Metronidazole has antiprotozoan and antibacterial action.
Active against Trichomonas vaginalis, Gardnerella vaginalis, as well as obligate anaerobic bacteria (including Peptostreptococcus anaerobius).

Indications and usage

- vaginal candidiasis;
- Trichomonas vaginitis;
- bacterial vaginosis;
- mixed vaginal infection caused by pathogens of bacterial vaginosis, vaginal candidiasis and trichomonas vaginitis.

Contraindications

- I trimester of pregnancy;
- lactation (breastfeeding);
- Hypersensitivity to the components of the drug and their derivatives.

Dosage and administration

The drug is prescribed for 1 vaginal suppository for the night and 1 vaginal suppository in the morning for 7 days.
With recurrent vaginitis or vaginitis resistant to previous therapy, the treatment should be increased to 14 days.
Vaginal suppositories should be inserted deep into the vagina using disposable fingertips contained in the package.
In the appointment of the drug in elderly patients (over 65 years), a change in dosing regimen is not required.

Adverse reactions

Local reactions: burning, itching, irritation of the mucous membrane of the vagina (2-6%).
Gastrointestinal: pain in the stomach cramping character (3%), metallic taste in the mouth (1.7%); in rare cases - dry mouth, constipation, diarrhea, loss of appetite, nausea, vomiting.
From the side of the central nervous system: headache, dizziness, ataxia, psychoemotional lability, peripheral neuropathy (with overdose and prolonged use of the drug), convulsions (less than 1%).
Dermatologic: skin rash (less than 1%).
Hemic and lymphatic: leukopenia (less than 1%).
Other: change in the level of glucose in the blood serum (when determined by the hexokinase method).

Pregnancy and breastfeeding

Appointment of Neo-Penotran contraindicated in the first trimester of pregnancy. In the II and III trimester of pregnancy, Neo-Penotran can be prescribed only under strict indications and under the close supervision of a doctor.
If necessary, the appointment of Neo-Penotran during lactation breastfeeding should be discontinued, because metronidazole is excreted in breast milk. Breastfeeding can be resumed 24-48 hours after the end of treatment.

With caution, the drug should be prescribed to patients with severely impaired liver function (including porphyria), diseases of the peripheral and central nervous system, and impaired hematopoiesis.
In the case of vaginitis caused by Trichomonas vaginalis, it is advisable to simultaneously treat the oral partner with oral metronidazole.
When prescribing Neo-Penotran, patients should be warned that alcohol should be avoided during the period of use of the drug and within 24-48 hours after the end of the course of treatment because of the possibility of disulfiram-like reactions. With the simultaneous use of Neo-Penotran and a contraceptive diaphragm or a condom, the basis of the suppository can interact with rubber.
The drug is not recommended for virgins.
Suppositories should be used only intravaginally, they can not be swallowed or used in another way.
Use in pediatrics
The drug is not recommended for use in children.

Overdosage

To date, no cases of overdose of the drug Neo-Penotran have been reported.
Symptoms Metronidazole is absorbed in sufficient quantity in order to cause systemic effects. In case of an overdose of metronidazole, nausea, vomiting, abdominal pain, diarrhea, itching, metallic taste in the mouth, ataxia, dizziness, paresthesia, convulsions, leukopenia, dark urine staining may occur.
In case of overdose of miconazole nitrate, nausea, vomiting, anorexia, headache, diarrhea can occur.
Treatment. In case of accidental ingestion of a large amount of the drug, gastric lavage and symptomatic therapy are performed.

Drug Interactions

With the simultaneous use of metronidazole with ethanol disulfiram-like reactions occur.
With simultaneous use of metronidazole with disulfiram may be a violation of the central nervous system (mental reactions).
With simultaneous use, metronidazole enhances the effect of indirect anticoagulants.
With simultaneous use with lithium preparations may increase their toxicity.
The simultaneous use of metronidazole with phenytoin increases the level of phenytoin in the blood, and the level of metronidazole in the blood decreases.
Phenobarbital with simultaneous use reduces the level of metronidazole in the blood.
With simultaneous use with cimetidine may increase the level of metronidazole in the blood.
Metronidazole and miconazole inhibit the metabolism of astemizole and terfenadine, as a result of which the concentration of astemizole and terfenadine in the blood plasma increases.
It is possible to change the concentration of theophylline and procainamide in plasma with simultaneous use with Neo-Penotran.
