Mechanism of action
Combined preparation with anti-inflammatory and keratolytic action for external use.
Betamethasone - a synthetic fluorinated glucocorticoid - has a pronounced anti-inflammatory, immunosuppressive and anti-proliferative effect with a slight mineralocorticoid effect. Reduces the formation, release and activity of chemical inflammatory mediators (kinin, histamine, liposomal enzymes, prostaglandins), inhibits the migration of leukocytes and macrophages to the site of damage, reduces vasodilation and increased permeability of the blood vessels in this area. Inhibits III and IV type of hypersensitivity reactions, while blocking the toxic effect of antigen-antibody complexes, which are concentrated near the walls of blood vessels, causing the development of allergic vasculitis on the skin. It also inhibits the action of lymphokines, target cells and macrophages that cause the development of reactions according to the type of allergic contact dermatitis. In addition, it prevents access of sensitized T-lymphocytes and macrophages to target cells.
Salicylic acid has a keratolytic effect, facilitates the penetration of GCS into the skin, also has a squamolytic, antimicrobial and antifungal effect, and also restores the normal pH of the skin.
Betamethasone
Suction and distribution
When applied to healthy, intact skin, betamethasone is absorbed to a small extent. Damage to the skin, the presence of an inflammatory process and the use of an occlusive dressing may increase absorption. Systemic absorption after external use is 12-14%. Plasma protein binding is about 64%. Vd - 84 l.
Metabolism and excretion
Betamethasone is biotransformed in the liver. Metabolites are excreted mainly in the urine and in small quantities in the bile.
Salicylic acid
Suction
For external use with occlusive dressing Cmaxsalicylic acid in plasma is reached in about 5 hours.

Most salicylic acid is excreted in the urine as metabolites: salicylic uric acid (52%) and salicylate-glucuronides (42%), in unchanged form - about 6%.
Indications and usage
Subacute and chronic dermatosis, accompanied by hyperkeratosis, sensitive to the treatment of GCS, including:
-psoriasis;
- neurodermatitis;
- lichen ruber;
- eczema (including numular, eczema of the hands);
- dishydrotic dermatitis;
-seborrheic dermatitis;
-ichthyosis and ichthyosiform changes.
Ointment or lotion is applied 2 times / day on the affected skin and lightly rubbed, in some cases it is enough to use the drug 1 time / day.
The duration of treatment should not exceed 3 weeks. If necessary, longer treatment therapy can be used less frequently (every other day).
To prevent recurrence in the treatment of chronic diseases, the use of the drug should continue for some time after the disappearance of all symptoms.
For children the drug is prescribed with great caution, for perhaps a shorter period due to the fact that with a larger area of skin in relation to body weight as compared to adults and insufficiently developed epidermis in children, absorption of proportionally more betamethasone and, accordingly, salicylic acid is possible and, therefore, manifestations of systemic toxicity. Application under diapers (especially plasticized) enhances absorption, because diapers act as an occlusive dressing.
If the time of use of the drug is missed, Belosalic should be applied as soon as possible. If the time of applying the next dose is approaching, the missed dose should not be used, with the next use there is no need to increase the amount of the drug.
Adverse reactions
Local reactions: burning, itching, folliculitis, acne, hypopigmentation, atrophy and thinning of the skin, stretch marks and hypertrichosis.
Systemic reactions due to the action of betamethasone:growth retardation, reversible suppression of the hypothalamic-pituitary-adrenal system, Cushing's syndrome, increased intracranial pressure, hyperglycemia and glycosuria.
Systemic reactions due to salicylic acid: pallor of the skin, fatigue, drowsiness, hyperventilation (accompanied by tachypnea), nausea, vomiting, hearing loss, confusion.
Systemic adverse reactions can occur with prolonged use, when applied on large surfaces of the skin or with the use of occlusive dressings.
Contraindications
- viral infections of the skin;

Perioral dermatitis

Increased sensitivity to the components of the drug.
Pregnancy and breastfeeding
Adequate and well-controlled studies of the safety of external use of GCS and salicylic acid during pregnancy have not been conducted. Therefore, the use of Belosalic is possible only in cases where the intended benefit to the mother outweighs the potential risk to the fetus. In such cases, the application should be short and limited to a small area of the skin.
During lactation, the use of Belosalic is possible only under strict indications, but at the same time the drug should not be applied to the skin of the mammary gland before feeding and applied for a long time on a vast surface. When using Belosalic, betamethasone and salicylic acid can enter the systemic circulation in quantities sufficient to be excreted in breast milk in detectable concentrations, but it has been established that, if the dosage regimen is observed, these quantities are safe for the baby.
ATexperimental studies It has been established that the external use of potent corticosteroids and oral administration of salicylic acid in laboratory animals has a teratogenic effect.
Special notes
In cases when hypersensitivity reactions (itching, burning and redness) develop at the first use of Belosalic, the use of the drug should be stopped immediately.
Do not use the drug on the skin of the eyelids and the periorbital area and prevent it from getting into the eyes.
Long-term use on the skin of the face is not recommended due to the possible development of rosacea, perioral dermatitis, facial skin atrophy and acne.
The use of the drug in the anogenital area should be avoided.
The lotion is particularly suitable for use on the scalp and other hairy areas of the skin; it is easily distributed over the affected area, does not lubricate or glue the hair, does not dry the skin, does not leave visible marks. Apply the ointment on the hairy areas of the skin is not recommended.
When using the drug in patients with impaired liver function and long-term therapy, especially with the use of occlusive dressings, consideration should be given to a possible increase in the absorption of active substances and the manifestation of the systemic effects of betamethasone and salicylic acid.
Overdosage
Symptoms: when applying the drug to large areas of the affected skin over a long period (more than 3 weeks), especially when using occlusive dressings, or with prolonged use in children, it is possible to increase the absorption of active ingredients into the systemic circulation and increase the systemic side effects associated with betamethasone and salicylic acid.
Treatment: drug withdrawal. If necessary, conduct symptomatic therapy. With a sharp cessation of the use of the drug and the appearance of symptoms of GCS withdrawal (fever, myalgia, arthralgia, weakness), a course of GCS replacement therapy for systemic use should be given.
Drug interactions
Use simultaneously with Belosalic cosmetic or dermatological preparations for the treatment of acne, ethanol-containing preparations for external use, medical soap with a pronounced drying effect may cause skin irritation.
Storage conditions
The drug should be stored at a temperature not higher than 25 ° C. Shelf life ointment - 4 years. Shelf life - 3 years.
