Mechanism of action
Listata is an inhibitor of gastrointestinal lipases. The mechanism of action is associated with the formation of a covalent bond with the active serine portion of the gastric and pancreatic lipases in the lumen of the stomach and small intestine. The enzyme at the same time loses the ability to break down fats, coming from food in the form of triglycerides, into absorbable free fatty acids and monoglycerides. As a result, the number of calories entering the body decreases and, consequently, the patient's body weight decreases.
After 24-48 hours after taking the drug, there is an increase in the concentration of fat in the fecal masses. After discontinuation of the drug, the fat concentration returns to its original level after 48-72 hours.
Indications and usage
Obesity and overweight (in combination with a moderately low-calorie diet).
Contraindications
Chronic malabsorption syndrome; cholestasis, hypersensitivity to the drug.
Pregnancy and Breastfeeding
It should not be prescribed during pregnancy, due to the lack of reliable clinical data confirming the safety of its use.
It is not established whether orlistat penetrates into breast milk, and therefore the use of Listat during lactation (breastfeeding) is not recommended.
ATexperimental studies no teratogenic and embryotoxic action of orlistat was detected.
Dosage and administration
The drug Listat prescribed 120 mg 3 times / day along with the main meals. The drug is taken with food or no later than 1 hour after meals. If the meal is skipped or if the food does not contain fat, you can skip the reception of Listat.
Increasing the dose above the recommended does not increase the therapeutic effect.
Adverse reactions
Gastrointestinal: oily discharge from the rectum, gas, imperative urge to stool, steatorrhea, increased bowel movement and fecal incontinence (as a rule, these phenomena are transient in nature and occur in the first 3 months of treatment); rarely, pain or discomfort in the abdomen, flatulence, loose stools, pain or discomfort in the rectum.
Allergic reactions: skin rash, itching, angioedema, Anaphylactic reactions.
Special notes
Against the background of therapy with Listat, the patient should receive a balanced, moderately low-calorie diet containing no more than 30% of calories in the form of fat. With a higher fat content in the diet increases the likelihood of adverse reactions from the gastrointestinal tract.
Reducing the body weight of patients can improve metabolism in patients with diabetes mellitus and require a reduction in the dose of oral antidiabetic drugs.
The safety and effectiveness of Xenical in patients with impaired liver and / or kidney function has not been established.
In clinical studies in most patients who received Xenical for 2 full years, the concentrations of Vitamins A, D, E, K and beta-carotene remained within the normal range. In some cases, for an adequate intake of all the nutrients in the body, you can assign multivitamins, which should be taken no less than 2 hours after taking Listatas or before going to bed.
Use in pediatrics
The safety of the drug in children has not been established.
Drug Interactions
There is no established drug interaction in the combined use of Listatas with Digoxin, Warfarin, Metformin, oral contraceptives, Nifedipine, statins and ethanol.
With the simultaneous use of Listat can reduce the absorption of fat-soluble vitamins.
Storage conditions
The drug should be stored in a dry place at temperatures not above 25 ° C.
