Clinical Pharmacology
Aziderm causes a moderate cytostatic effect (due to inhibition of the synthesis of cellular proteins), manifested in the normalization of keratinization processes in the wall of the follicles of the sebaceous glands; It has an antimicrobial effect (primarily against Propionibacterium acnes and Staphylococcus epidermidis) both on the skin surface and in the sebaceous glands, as well as an anti-inflammatory effect (reducing the metabolism of neutrophilic granulocytes and the production of free radical oxygen forms that are important factors in maintaining the inflammatory process).
With prolonged use of the drug is not formed resistance of microorganisms. Azelaic acid inhibits the growth and viability of abnormal melanocytes in a dose-dependent manner. The therapeutic effect of hyperpigmentation (melasma) is due to the inhibition of DNA synthesis and / or the suppression of cellular respiration of abnormal melanocytes. The therapeutic effect of azelaic acid in rosacea is associated with a decrease in the formation of pro-inflammatory reactive oxygen radicals. After applying Aziderm cream to the skin, azelaic acid penetrates the epidermis and dermis.After 1g of azelaic acid (which corresponds to 5g of cream) is applied to the skin, about 4% of the dose is absorbed into the systemic circulation. Part of the absorbed azelaic acid is excreted by the kidneys in unchanged form, and some as dicarboxylic acids (C5, C7), which are formed from azelaic acid as a result of beta-oxidation.
Indications
Acne vulgaris (acne vulgaris), pathological hyperpigmentation, incl. melasma.
Dosage
Outwardly. The maximum duration of therapy depends on the individual picture of the disease and the severity of the symptoms and is determined by the attending physician. Adults and children over 12 years old apply the cream evenly with a thin layer 2 times a day (morning and evening) on previously cleaned (with mild cleansers and water) and dried areas of the face and, if necessary, on the neck and upper chest affected by acne. For the entire surface of the face, about 2.5 cm of a tube of cream tube is sufficient.
The drug Aziderm should be used regularly throughout the course of treatment, the duration of which depends on the severity of the disease. The minimum duration of treatment for melasma is 3 months. During the treatment of melasma, sunscreen (UV B and UV A) must be used to prevent exacerbation of the disease and / or re-pigmentation of the skin under the influence of the sun. In patients with acne vulgaris (acne vulgaris), marked improvement is usually observed after 4 weeks.For best results, use of the drug should be continued for several months.
Contraindications
Hypersensitivity to drug components, children's age up to 12 years.
Special instructions
The drug should be avoided in the eyes, as well as on the mucous membranes of the nose, lips and mouth. If this happens, rinse immediately with plenty of water. In cases of severe skin irritation in the first weeks of treatment, Aziderm can be applied 1 time per day or reduce the amount of cream applied to the skin; short-term withdrawal of the drug is also possible. After the disappearance of the symptoms of skin irritation should resume regular use of the drug in the recommended dose.
Dermal melasma is insensitive to drug treatment. For carrying out differential diagnostics use Wood's lamp. Aziks-Derm can be used in combination with other methods of treatment of acne. In this case, a correction of the dosage regimen of simultaneously used drugs is possible. Aziks-Derm has no effect on the reaction rate when driving or working with other mechanisms.
Side effects
On the part of the skin rarely (mainly at the beginning of therapy): skin irritation, skin flushing, erythema, burning sensation, itching, peeling, which disappear independently during further treatment. Allergic reactions (eg rash) may occur.After applying the cream to the skin, a burning sensation and tingling may occur, which disappears within 15 minutes.
