Mechanism of action

OSTEREPAR - an inhibitor of bone resorption. Sodium elendronate is a bisphosphonate, a synthetic analogue of pyrophosphate that binds bone hydroxyapatite. Being a non-hormonal specific inhibitor of osteoclast activity, it prevents bone resorption. Does not affect the formation of bone tissue. Stimulates osteogenesis, restores a positive balance between resorption and restoration of bone. Progressively increases bone mineral density (regulates phosphoric-calcium metabolism), promotes the formation of bone tissue of normal composition and structure.

Indications and usage

Treatment of postmenopausal osteoporosis in order to prevent spinal and hip fractures.

Contraindications

- anomalies of the esophagus and other factors impeding the patency of the esophagus (including achalasia, stricture);

- the inability of the patient to remain upright, even sitting, for 30 minutes;

- hypocalcemia;

- chronic renal failure (CC <35 ml / min);

- pregnancy;

- lactation period (breastfeeding);

- childhood;

- vitamin D deficiency;

- severe violations of mineral metabolism;

- hypersensitivity to the drug.

The drug should be used with caution in diseases of the digestive tract in the acute phase (dysphagia, esophageal diseases, gastritis, duodenitis, ulcer, serious gastrointestinal disease in the preceding 12 months, for example, peptic ulcer, Gastrointestinal bleeding, surgery, except for spastic surgery pylorus of the stomach).

Dosage and administration

The drug is taken orally. The recommended dose is 70 mg once a week. To ensure optimal absorption of the drug should be taken in the morning on an empty stomach, 2 hours (at least 30 minutes) before the first meal or liquid, drinking plain water. Other drinks, including mineral water, food, a number of medicines can affect the absorbability of alendronate.

In order to avoid local irritation of the oral mucosa and esophagus in the morning, immediately after getting out of bed, drink at least 200 ml of plain water, then take a pill without chewing and preventing it from dissolving in the mouth. For the next 30 minutes you cannot take a horizontal body position after this period should take breakfast. You can not take a pill in the morning, before getting out of bed or in the evening, after going to bed.

Treatment with alendronate should be supplemented with Calcium and vitamin D.

Elderly patients dose adjustment is not required.

When QA> 35 ml / min dose adjustment is not required. In case of renal failure with CC <35 ml / min, it is not necessary to prescribe the drug because of the lack of clinical experience.

Drug Interactions

Calcium, antacids, some oral preparations, food, drinks, incl. mineral waters affect the absorption of alendronate. Medicines can be taken orally not earlier than 1 hour after taking alendronate.

Other types of interactions, with the exception of changes in absorption, are unlikely.

Ranitidine increases the bioavailability of alendronate (clinical significance is unknown).

NSAIDs increase the adverse effects of alendronic acid.

Special studies on drug interactions were not performed, however, patients who took other oral medications at the same time participated in the studies with alendronate. At the same time, they did not observe adverse reactions associated with the simultaneous intake of other drugs.

Overdosage

Symptoms: hypocalcemia, hypophosphatemia, adverse reactions from the upper gastrointestinal tract (heartburn, esophagitis, gastritis, ulcer).

Treatment: there is no specific treatment. Recommended intake of milk, antacids. In order to avoid irritation of the esophagus, one should not induce vomiting, the patient should take an upright position (standing or sitting).

Storage conditions

The drug should be stored out of the reach of children, dry, protected from light, at a temperature below 25 ° C.
